When Obamacare’s individual mandate takes effect in 2014, all Americans who file income tax returns must complete an additional IRS tax form.

1. Determination of “qualifying” health insurance.
Taxpayers must demonstrate that they obtained qualifying health insurance for each month of the year in order to avoid payment of this tax penalty.

2. Disclosure of personal identifying health information.
3. Exemptions from Individual Mandate: Prisoners, Undocumented Immigrants, Welfare Recipients.

4. IRS penalties and interest on unpaid mandate taxes. Because the Affordable Care Act’s individual mandate penalty is a tax, the IRS will be able to assess interest and non-criminal penalties on those families who will not or cannot pay the tax.

No it isn't the official IRS form.

But the information regarding 'the form' is important. We will all be on Medicaid! Small Business owners will have to close their doors because of the penalties that will be put on us.

Anyone thinking they will still receive the same health care that their insurance gives them is WRONG. Read and analyze.
