Well, what were you wearing on those other 4 dates? Where something similar to that. But I agree with mir86 though, you should wear based on how you want the outcome of the date to turn out.

Yes, definately depends on how far you want him to go. Bring a sweatshirt at first to make sure it's not freezing in there then I'd probably go with a long sleeve shirt with a t-shirt on top. Definately jeans and sneakers. Lots of deodorant because going over to a guys place to "Watch a movie" means screwing with eachother during the movie, (i.e. tickling, etc)
