Rockford, Illinois (metro population 350,000):

We have Chicago Rockford International Airport with flights to Denver, Phoenix, Las Vegas, Tampa, and Orlando. Our international flights are to Mexico, the Dominican Republic, and Jamaica.

Also, we're one of the busiest air-cargo airports in the country. UPS operates their 2nd largest airport hub in the United States at RFD.

Naw. John Wayne is a pretty small airport.

I would not call Klamath Falls International Airport an "International Airport" It is in that if it is called "International" it has Immigration and Custom officers at the airport or near by on call but there is no internaitonal passenger service there.

Small metros with International Airports that come to my mind.

Fort Myers: Non-stop service to Canada and Germany
Salt Lake City: Delta has a hub there.
Fairbanks: Seasonal flights to Europe
Anchorage: Has a few international flights especially charters in the summer to Europe and Asia and it is the 3rd largest air cargo hub in the country.
Raleigh: Non-stop flight to London on American Airlines, flights to Canada.
Portland Maine: It has service to Canada.
Palm Springs: Service to Canada
Guam: Service to other countries in Asia. Guess we can include Guam
