While I usually think of the midwest as middle america, to me the states of Nebraska Iowa Kansas and Missouri are middle America. Particularly the cities of Omaha, KC, Des Moines, Lincoln, and Wichita and a few other nearby cities
Middle America is just your average Joe living away from cosmopolitan areas such as East Coast, West Coast and Southern Florida.
Pretty much this. It's not referring to a specific geography. It's kind of a deragatory term that basically means bland. It's referring to people who live in subdivisions, shop at Target/Wamart/Macy's, spend their weekends at church and coaching Junior's soccer team, drive some kind of SUV or pick-up truck and have hobbies like scrapbooking and woodworking. It's Bud Light (vs. craft beer), Friday night football games (vs. the theater) and McDonald's (vs. artistanal toast.)
Middle America = middle class, average education levels, lack of sophistication, families, conservative, boring geography and weather, white, christian, sparsely populated
not Middle America = rich, famous, powerful, artists, liberal, diverse, vacation spot, young singles, high education levels, densely populated, less religious
Somewhere in the Midwest, where people are working/middle class, have the most neutral accents, the most natural weather, most moderate religiosity, most people are white? Not very urban, rural and suburban areas apply. Flat landscape, no ocean, perhaps a great lake.

I think of Middle America as the Sarah Palin/Honey Boo-Boo demographic

but.. I also think of Middle America as in ignorant America, not ignorant in a negative sense of the word but less experienced in life outside of their bubble and more sheltered resulting in believing in such things like the rapture and Trump "Make Murica Great Again".
