The inside of my Bilco doors the concrete that covers the frame is cracking and needs to be redone. My first thought was just clean it up and put some fresh concrete over it. However, im pretty sure concrete isnt going to adhere to it too well.

What are my options?

Concrete will stick, especially if you wet down the existing stuff and use a latex modified product.

That said you have a couple of problems that ought to be addressed.

Firstly there is mortar missing from between / behind the concrete block that ought to be supporting this door. You really gotta getta address that, even it that means removing the doors, backfilling the mortar and doing this right. Otherwise some kid or a diligent ground squirrel will be knocking in that unsupported door and unleashing all kinds of havoc in your basement.

Secondly 99 times out of a hundred when there is the kind of surface cracking that is visible in the photos you posted the root cause is ICE -- you gotta address that or the new concrete will just be busted out by the inevitable force of water freezing. Maybe there is water dripping on the opening from above or maybe enough snow melts on the doors to lead too the problem. Maybe you need gutters or longer down spouts or better insulation or all of the above BUT unless you address the root cause your patch will be blown apart by ice in a season or three...
