How do I obtain the NYS depart of labor history sheet they are requesting??? I can't find it I created the account and everything already??? Help please
@BoriX15 If it doesn't apply to you then, you don't need to fill it out but they do want to know how were you supported if you have any unemployment dates. If someone supported you during those dates, you need a letter from them stating that and they must get it notarized.

Thanks yeah I just have to write the letter for my dad to sign. I was only unemployed luckly for a few months after high school. I know these ppl taking forever. What happens if they don't contac u within the 30 days. What do we have to do?

Nope nothing. Next week I'll make a month of having done my medical n emailing my BI n still no response. So it's been 30 days @wrestler55 n still nothing. U should call the supervisor or something
