What are they? Why haven't they told us what they are in advance?

Get rid of the lock insurance companies have over healthcare. Let's go back 15 years to when insurance just covered hospital stays. Then just watch how quickly the everyday costs come down. When you have to pay out of your own pocket you are going to pay closer attention to what a doctor visit costs and will shop around. When you only pay $15, you don't care, nor do you care how much all the tests cost or even if they are necessary.
