I also don't answer the door to strangers, fortunately, at our current home, we are unlikely to have any. Workmen? Sure.
I can only remember once having a child ring the bell to ask ( at a previous home), and I let them in.

I work for a non-profit, so we see homeless and low-income clients on a daily basis. There are restrooms for their use, but only during office hours. Our thrift store is open evenings and weekends, and there is no access to the public facilities then.
We have signs posted that say "no public restroom" in multiple languages, but the reality is, we don't deny anybody the use of the staff bathrooms. And yes, we've paid the price, they can get pretty gross. It isn't fair to the employees or the volunteers working, but there is no easy answer.

If a worker is already in my home to do some sort of work, I would certainly allow them to use the guest bath, or powder room. That's what it's for, people whom I've invited into my home as a guest or employee. I don't allow them into my master bath however. That's private.

I also don't keep any medications in the guest bath. We've all seen the comedy tv shows about medicine cabinet snoopers! After I had surgery once, I was on pain medication and I left my pill bottle on the kitchen counter. I went into another room for a few minutes while we had a "friend" over. The next time I went to get my medication after they left, a third of the pills were missing. :^(

Absolutely not at my home.

I worked a short time for a psycho lady who would take supplements to clean her out and she would have to often stop at strangers' homes. She liked to tell us about it. Ugh.

We were nice and let kids/adults use our employee restroom, at first. But, it's too worrisome, liability wise, it's not set up for the public, we store cleaning chemicals and personal items there, have to walk by machinery, computers and sharp objects, etc. We just say no now.
