Some photos are more flattering than others. I wouldn't take it as a critique of your real-life appearance.

I get this quite a bit from people trying to be polite.

I'm overweight and not particularly photogenic. I'm not ugly, but I know I'm not going to be a dream photography subject. Don't blow smoke up my rear end.

I am not photogenic and I know it! If someone says 'that's a great picture of you,' I take it in the spirit I'm sure it was intended: a compliment!!!

Idk about others' motives, but I've said it a few times and meant nothing more than what I said--it's a good picture. The person looks good in it.

Looking for hidden insults (that aren't even there) is a sign of low self-esteem or paranoia in my opinion. I'm un-photogenic and generally hate how I look in photos, so if someone says it's a good photo, I take it for the compliment that it is. It means I look good in the photo, that's all.
