Re: back child support. It is absolutely possible that he could get hit up for back child support. I'd say it's pretty unlikely he'd have to at this point, but then again, I've seen some pretty stupid judges.

Depends on the state. In my state child support is only payable from the DATE OF FILING on. No retroactive support. Also in many states the legal father is the husband regardless of whether or not he is the bio father. So if this woman was married when her child was born hopefully her husband is the legal father. Also, this kid is definitely 18 or close to it so hopefully your sister's husband is off the hook.
