I hope everyone is well! My name is Matthew Cat. I am 45 years old, a single parent, currently unemployed, divorced, and have a five year old child named Alex, who was diagnosed last month, May, with autism. Currently, my friends are doing a survey to better understand the parents with kids with autism. As a father with an autistic son myself, I've been helping them with outreach and connecting to other parents. I understand if you do not wish to take it as I, too, am concerned for my privacy and confidentiality. It would be great if you did, however. You do not need to answer all the questions, if you are uncomfortable answering them; you can put 'N/A' for those you do not wish to answer.

Can you provide a proper link, not a shortened version?
Would also help if you could tell us a bit what the survey is about (feelings of parents? difficulty parents have? severity of autism? what?), and what the results will be used for (scientific research? products for autistic kids? a book? pure curiosity?)
Seems odd that your child was diagnosed a month ago, at 5, and your first reaction is to conduct a survey. Are you sure this isn't far a class or something?
