Work search record (apply, EDD, fill, appointment)

California EDD sent me a work search record that I have to fill out and return during my in-person appointment. I usually apply online but the form asks for "Person Contacted". Is it sufficient to say "Applied online"?

Also for the "Results" section, what is the proper way of saying never heard back from the employer. Or should I just say "resume submitted"

You can say, "nothing," "no response," or "silence." Take your pick.
