If it was me and I had a good relationship with the boss, I'd give him a call first, then follow up with the email afterward. And if I'd had a bad relationship with the boss I'd do it the same way. But I'd enjoy the phone call more.

I'm just trying to do this politically correctly as I work in a field where everyone knows everyone.
I think I'll call him first thing Monday...and email him with HR, and his boss copied simultaneously.
give him or her the courtesy of direct contact before bring in HR... most likely once you contact him or her he or she will contact all appropriate parties who need to be notified within the company....
