I know it comes up a lot, but I don't believe in the "Peter Principle." Rather it seems that people who are incompetent at their current job get promoted more often than competent people. Maybe management wants to keep competent people actually producing or more likely doesn't want to compete with them once they get promoted because the competent will outshine them.
Competent people hire competent people or people who are smarter than themselves. However incompetent people don't want to be compared with the competent so they hire those who are less competent than themselves. It only takes a few cycles of this for most of management to be filled with the incompetent.
Politics. Most American companies are a social club/frat house/sorority and being able to smooze well is more valueble than doing your job well or adding value to the company. It happens with hiring, promotions, raises, and layoffs and HR fully endorses it because "fit" is the most important thing. As a result, the best employees need to keep job hopping to get the raises and promotions they deserve.
In my case it was definitely politics. The new "Boss" was the head of the labor Union's daughter. There were 2 of us in the office that didn't get our jobs because of nepotism or politics.
1) Connections. This one is common. For example, an idiot engineer I worked a bit with years ago is the son of VP. So, naturally, he's promoted as fast up the chain as possible, making VP by 40. Nobody knows what the guy does other than fluff up his resume and ego, but there you.
2) Favors: The classics - sleeping with the boss, money, etc.
3) Brown-nosing: Another favorite. The boss's butt-buddy, who oozes around, praising everything he does, and thus doesn't have to do any real work or even be held accountable for anything.
4) Mass stupidity: When most of a department is stupid or otherwise useless, it's easy for idiots to rise in the ranks. The worst position is to be one of the sane workers in such a department since you'll do all the work and, oddly enough, be blamed for "not fitting in" because you're not useless!
Thankfully, my current department at work is free of this nonsense, but other departments in the company have some of it, and other companies I've worked at were literally ruined by it.

This should be the standard reply to any "Why did person X get a promotion when . . . " type threads.
When will people ever understand ability has absolutely nothing to do with hires and promotions?
