When I worked for the state of Oregon the policy was a doctors note if longer then 3 days. I had one guy who use to call in drunk. Fortunately that didn't last too long. he was also using meth. He was called to Salem for counseling and threw some stuff at the counselor. She said "Thank you Mark, that will be all."

Our company has personal days. No longer medical call in. Once we use up the allotment then should it be for a medical injury or condition we are asked to render proof. Imagine calling in to say...I'm in the hospital can I send the bill over to you guys as proof of why I'm in the cardiac unit ?

After the 3rd day of sick leave, a doctor's note is required upon returning. Otherwise just a phone call, text, or email to the supervisor is needed. If it's discovered that someone was not really sick (not likely), we would reprimand them and change their time from sick leave to vacation day(s).

It is manager discretion. Rule of thumb is Dr. note for 3 consecutive days.

Same here, more than 3 consecutive days and need a Dr's note. Earlier this year I went out with Bronchitis for a week just about.

In 2 years, never took much vacation/sick time. I went back after 2 days and tried to grit it out, made it 1 more day and had the MD come to my work and diagnose me with bronchitis. I went home and was off 2 more days and finally went back the next Monday. Doctor wrote me a note to stay home until the next Monday~

We don't have "sick days" or "vacation days", it's all lumped together as PTO (paid time off). They don't really care if you're at the beach or at the hospital, and they don't care if you have to cancel your planned beach trip because you had to spend time in the hospital. Unless you get into a short term disability situation, where an insurance policy would kick in, nobody asks for doctor's notes.

PTO, more than five consecutive days of an unplanned absence turns into a leave of absence request. Five instances (not five days) of unplanned absence are allowed before the disciplinary process kicks in. Basically you can take up ten absences before you're terminated for excessive absenteeism. A doctors note isn't required for anything other than requesting the leave of absence if you are out for longer than five consecutive days, it's not accepted as an 'excuse' for missing time.

Personally I think it's a pretty generous policy, but we still have plenty of people that manage to get fired for calling out too much.
