I'd argue Vatican City is basically a district of the much larger city of Rome, even if legally it is its own country and city.

Reykjavik. It has basically what every large city has and its metro area is only 200K.

I was thinking possibly Reykjavik, as it is the biggest city in over 1,000 km and only has 200,000 in the metro

That has to Vatican city.
Not only is it small, but it is a capital and a state. It has greater influence in the world through the Catholic church than all but a few Major cities.

With all "elit" who came there, we can consider it as a city who has a real impact while being really small.
