It's been a couple of years when I was in therapy at HealthSouth in Rock Hill. At that time the Amendment Ave location in Rock Hill sent a therapist to a York office several days a week. I do not know if they still provide this service for York you might want to call and inquire.

depending on the part of York, there is Smith Physical Therapy/ Sports Medicine . One is located in Ft Mill on 160 and the other is on HWY 49 in York near the Walmart. I saw Jesse Smith at the Ft Mill location, and he was wonderful, and gave me the tools I needed to keep myself going and out of pain. I cant recommend them highly enough.
