    Observation: The seal on the opening of the ice machine is visibly soiled.

    Observation: There is not a person on staff who has a Certified Food Protection Management certificate. Establishment was required to have the certificate by January 23, 2015.

    Observation: The fans in the reach in refrigerator in the kitchen are heavily soiled.

    Observation: The walk in coolers ceiling and walls are heavily deteriorated with pieces of the ceiling hanging down. The ceiling and walls are also moderately/heavily soiled.

    Observation: There were frozen raw steaks that were in a on-site reduced oxygen package. There were frozen cooked pulled pork that were on-site reduced oxygen package. Corrected on site.
