Observation:thermometers on outside of walk in cooler and walk in freezer are not accurate.

Observation:Potentially hazardous ready to eat food not in their original container were not properly labeled.

Observation:Several items in walk in refrigerator, 3 door fridge has no date mark.

Observation:Bagged ice offered for retail needs store information on each package for trace back.

Observation:utensils need to be stored with handles facing up to avoid contamination of food contact part of utensil.
- Protecting food from environmental contamination
Observation:Potatoes on the floor of the walk in cooler. Pickles and onions in self serve container with no lid.
- PHF/TCS food maintained at 135°F or above, except during preparation, cooking, cooling, or when time us used as a public health control
Observation: Pizza hot hold out of temp. Tempted at 122 top and 115 degrees on the bottom with my stem thermometer.

Observation:Milk and cheese in cooler at 42.2 on my digital stem therm.

Observation:several moldy/spoiled items from walk in today.

Observation:The ventilation hood above pizza oven is very dirty/dusty.
- Package integrity
Observation: one box of cereal removed for torn package/out of date, two large dented cans removed from service. Torn bag of ice in freezer need removed from service.
