Observation: Dented cans (tomato sauce and green beans) on can shelve. Must discard or return any can that is dented on top, bottom, or side seals. Also, any can that cannot sit level on shelf without wobbling. Removed voluntarily .Corrected on site.

Observation: Raw eggs stored above ready to eat food (RTE) in reach in cooler. Food shall be protected from cross contamination by storing eggs below, separating raw animal food during storage, preparation, holding, and display. Corrected on site.

Observation: Paint peeling away and damaged above dishwashing machine. Walls, wall coverings, and ceilings s must be designed, constructed, and installed so they are smooth and easily cleanable. Suggested to cover with fire retardent plasticboard or paint to protect from futre damaging.
- Physical facilities maintained clean: Cleaning frequency, restrictions, and methods
Observation: Air-duck vent is rusty above reach-in cooler in the kitchen. Physical facilities air ducks must be cleaned as often as necessary to keep them clean so they are not a source of contamination by dust, rust dirt ,and other materials. Repaint with light colors or replace it.
- Date marking for ready to eat, PHF/TCS food prepared on-site or opened commercial container held for more than 24 hours
Observation: Several food boxes have no date marks on food in the reach in coolers in the facility. Must date mark all ready to eat food with date opened/prepared and then discard within 7 days. Food may be discarded if observed without a date mark in the future.

Observation: One wet wiping cloth not held in sanitizer. Wiping cloths used to clean food contact surfaces shall be maintained in sanitizer. Owner educated and verbalized understanding. CORRECTED ON SITE.

Observation: The left gasket on the small reach in freezer is torn and in disrepair. All equipment must be maintained according to manufacturer requirements. Repair as needed, REPEAT VIOLATION

All potentially hazardous foods must be dated when cooked, prepped or opened. Date items such as cut melon, milk, OJ, and boiled potatoes for hash browns.
