4-302.14 - --There were no test strips available at the bar three compartment sink.
KDA_3_10111 - --There was an opened box of hot dogs located in the Coke reach in cooler (RIC) that did not have a date mark. The employee did not know when the box was opened. COS- discarded.
KDA_3_30412E - --There were three ice scoops and a cutting board being stored as clean on top of the ice machine. COS- moved.
KDA_3_30511A1 - --There is a bag of chips on the floor under the beer shelf in the storage room.

KDA_4_60111A - --Located in the storage room, there were several utensils stored as clean that were soiled with dried food debris. COS- moved to wash area.

KDA_4_60212B - --The microwave door cavities were soiled with dried food debris. Employee said that the microwave is cleaned every couple of days but was not sure of when the last time it was cleaned.
KDA_4_90311A - --In the storage area in the hallway, there was a box of paper cups stored on the floor. Located under the counter with the roller grill, there was a case and a sleeve of 12 ounce cups and a case and a sleeve of food trays stored on the floor.

KDA_5_20515B - --There is a leak on the mop sink faucet when the water is turned on.

KDA_6_501111A - --There were 15 fresh rodent droppings in the cabinet above and the counter beside the hand washing sink. There were 2 fresh rodent droppings next to the three compartment sink. There were 12 fresh rodent droppings under the counter below the three compartment sink. There were 10-15 ants in the soda nozzle holder at the downstairs bar.
KDA_7_10211 - --In the bar area, there was an unlabeled container of yellow liquid. Employee indicated that it was an all purpose cleaner. COS- labeled.
KDA_Defacto - --Coke RIC hot dogs 38F --Roller grill hot dogs 131F, 132F Employee indicated that the hot dogs had only been on the grill for 1.5 hours.
