COS - stopped her and explained that she needed to take her gloves off, wash her hands, and put on fresh gloves after handling dirty dishes. The pizza dough was discarded. The employee washed her hands and put on new gloves.
  • KDA_3_10111 - --In the make table there is an opened tub of ready-to-eat hot sausage that has no date on it. The kitchen employees are unable to say when this was opened. COS - discarded. In the make table there is an opened tub of ready-to-eat beef in bbq sauce that has no date on it. The kitchen employees are unable to say when this was opened. COS - discarded. In the make table there is a cut head of lettuce with no date marking available. The kitchen employees are unsure when this lettuce was originally cut. COS - discarded. In the make table there is part of a sliced tomato that is not dated for when it was cut. The kitchen employees are unsure when it was done. COS - discarded.
  • KDA_3_30212 - --In the kitchen there are unlabeled items in the donut area that were identified as being icings, coconut, and powdered sugar.
  • KDA_3_30412A - --There are cups that are used as scoops in the beef and sausage of the make table. The scoops do not have handles and are being stored in direct contact with the food items.
  • KDA_3_30412C - --There are two ice scoops for the ice machine being stored inside of the hand sink in the back room. There is visible dirt/grime inside of the hand sink. Let the manager know these needed to be cleaned.
  • KDA_3_30511A1 - --There is a package of pizza crust mix stored directly on the floor in the kitchen. There is a box of cheese bread sticks stored directly on the floor in the walk-in-freezer.
  • KDA_3_50118A1 - --In the reach-in below the make table there is a tub of ready-to-eat hot sausage that is dated as being opened 10/29. COS - discarded. In the reach-in below the make table there is a tub of ready-to-eat sauce that is dated 10/28. COS - discarded. In the walk-in-cooler there is a tub of ready-to-eat roast beef that is dated 11/10 to 11/22. COS - discarded. Made sure they understood it had been dated for too long.
  • KDA_3_50118A3 - --There are two chicken salad sandwiches in the fresh deli cooler on the retail floor. They are dated to discard on 12/18. The manager checked their charts of when things were made and said these chicken salad sandwiches were made Monday (11/16). COS - she had the dates corrected.
  • KDA_3_60211B4 - --There are bags of ice in the freezer that are for sale. The business information has Casey's Retail Stores, Ankeny, Iowa listed. It does not specify that the ice is coming from this specific store. Letting manager know that they should write their store number on the bags so that it can be traced back to the store it is coming from.
  • KDA_3_60211C - --There is a bulk display case on the retail floor for self service that has donuts, brownies, etc inside. There are no ingredients/allergens posted on or near the case. Asked the manager if she would be able to provide an ingredient/allergen list if a customer with allergies requested it. She said she did not believe she had one.
  • KDA_4_20211A2 - --There are three cracked plastic bowls on a shelf in the kitchen. Let the manager know that these need to be replaced. The ice bucket on the wall in the back room is duct taped on the bottom. Pulling the duct tape back revealed that the container is chipped and has holes in the bottom. Let the manager know that this needed to be replaced. She said they no longer use it anymore. Let her know it could just be removed in that case.
  • KDA_4_60111A - --There are dishes on the shelves above the 3 vat sink. A kitchen employee said all dishes on these shelves were clean. There is a bowl that has a spot of food residue remaining on the outside of the bowl. There are three lids that have sticky residue remaining on the outside of the lids. COS - moved to ware washing. There are rubber spatulas stored in a drawer in the kitchen. This was identified as clean dish storage. The rubber spatulas are sticky and grimy along the handles. COS - moved to ware washing. There are lids on a table by the fryer. They were identified as being stored as clean. There is a sticker on the outside of a lid stored here. Let them know this needs to be cleaned. In the walk-in-cooler there is a large pink/red container with bags of bread stored inside. There is a sticker on the outside of this container that was identified as remaining from the manufacturer's original labeling. Let the manager know that this needs to be cleaned off.
  • KDA_4_60111C - --There are crumbs/debris around the pans on the table by the fryer. There is dried sauce splash on the inside of the lid of the make table. The doors of the make table reach-in have food grime on them. The area around the soda nozzles on the soda pop machine has accumulated soda splash on it and mold growth. There were five rodent droppings in the cabinet where the creamers are stored. They do not appear to be fresh.
  • KDA_4_60211E4 - --There is slight pink mold growth on the edge of the white ice guard inside of the ice machine.

The containers are not labeled for direct food contact. The food is in containers inside of these containers, or in bags inside of these containers. --There is a can of Raid Ant & Roach Spray (EPA Reg 4822-596) in the storage room by the walk-in-cooler. The manager said that she believed this was used outside only, and hasn't been used for some time. --There is a spray bottle of bleach water. There are no bleach test strips. Upon asking, one kitchen employee said she believes this is used to clean the food prep counter in the middle of the kitchen. The manager said that it is not used for this, but she is not sure who made it or what they made it for. Let them know that if it is used on food contact surfaces, they need to have chlorine test strips. They are able to provide quaternary ammonia test strips for the sanitizer used in their sinks. --There is a tub of cut lettuce in the walk-in-cooler that is not dated. The manager said it was done yesterday (11/17).
  • KDA_KSA65_657a - --On the retails shelf there was Dayquil Severe (4 Cap) that was expired. Lot: 4097686401 Exp: 7/15. COS - this was removed from the retail shelf.
