- KDA_3_10111 - --A rotten decade watermelon was stored on a shelf under a prep table. COS, watermelon discarded.
- KDA_3_30111B - --Food employee was using bare contact with ready to eat fried taquitos to put into a serving pan for the buffet. COS, taquitos were discarded.
- KDA_3_30211A1a - --Raw pork was stored on top of a box of broccoli in the walk in cooler. Raw beef was stored on top of white container with sliced raw carrots in the walk in cooler. COS, raw meats removed.

- KDA_3_30511A1 - --Boxes of soda syrup was stored on the floor in the back storage area. A box of melon was stored on the floor in the back storage area. A bin of with raw pork was stored on the floor of the walk in freezer.
- KDA_3_50114A2 - --7 black storage bins of cooked chicken 63F were stored on the floor in front of the walk in cooler. PIC stated the chicken had been cooked the night before closing (10 P.M). The containers of chicken never went into the cooler. PIC stated the cooked chicken was leftout for cooling overnight. COS, cooked chicken discarded.
- KDA_3_50116A1 - --Steamed dumplings 114F were on the steam table. PIC did not know how long the steamed dumplings had been on the steam table. COS, steamed dumpling removed and discarded.
- KDA_3_50116A2 - --A cut head of cabbage 66F was stored in a box of whole cabbage. COS, cabbage discarded.
- KDA_3_50118A2 - --A container with breaded fried chicken was in the reach in cooler with date marking label of 7-16-15. PIC stated the actual prepared date was 8-18-15. COS, chicken dated with the correct date marking. An package of del ham was in the walk in cooler without an opened container date. PIC state the ham was opened on n8-16-15. COS, ham dated.
- KDA_4_10111A - --Raw breaded shrimps were stored in a blue non-safe material container. COS, raw shrimps removed.
- KDA_4_60111A - --Dried food debris was on a spoon that was stored as clean in a storage drawer. Dried food debris was on the blade of knife that was stored as clean on knife wall strip. COS, items removed for cleaning and sanitizing.
- KDA_5_20511B - --Food debris was in hand sink. A food container lid was stored on top of the handsink. COS,handsink cleaned and,food container lid was removed
- KDA_5_20515B - --Water faucets on prep sink and 3 compartments sinks leaks water. Drain pipes under the warewash machine leaking water onto the floor.
