- KDA_3_30211A1b - --In the RIC below the MT a pan of raw fish and a pan of raw chicken were stored on a shelf over RTE pasta noodles. COS, pasta moved to an approved location.
- KDA_3_30212 - --On the top shelf in the prep room there was a container of white powdery substance. The manager said it was flour.
- KDA_3_30611 - --At the salad bar containers of croutons, bacon bits, and sunflower seeds were not protected or guarded. COS, lids placed on the containers.
- KDA_3_50116A2 - --A container of butter stored on the counter next to the steam table had a temperature of 64 F. The manager said it was placed on the counter at 1:30 PM. COS, moved to the RIC. Also, there was a bowl of potato salad on the salad bar that temped 48 F at the top and 41 F at the bottom. The salad bar uses ice to keep cold. The bowl was resting on top of the ice. The manager said the potato salad was filled at 2:00 PM. COS, bowl pushed further down to be surrounded by ice.
- KDA_3_50118A2 - --In the Coca Cola RIC a container of cottage cheese was not date marked. The manager said it was opened yesterday, 1/19, in the morning. COS, dated.
- KDA_4_60111A - --In the prep room a the can opener blade, the meat slicer blade, and the meat tenderizer stored as clean had dried food debris on them.
- KDA_4_60111B - --There is an accumulation of grease in the hood vents.
- KDA_5_20314 - or, Installing an APPROVED backflow prevention device as specified under § 5-202.14.
- KDA_5_50117 - --There is no covered waste receptacle in the women's toilet room.
- KDA_6_30114 - --There is not a sign to notify employees to wash their hands in the women's toilet room.
- KDA_6_50112A - --There is grease buildup on the floor below the fryers. There is debris below and behind the equipment in the kitchen. There is an accumulation of debris below the grill. There is mold on the wall around the warewashing machine.
- KDA_7_10211 - --A spray bottle of clear liquid was not labeled. The manager said it was disinfectant. COS, labeled.
- KDA_7_20111A - --In the service area by the salad bar spray bottles of disinfectant were stored on a shelf next to individual containers of creamer, napkins, and single service containers. COS, moved. In the prep room boxes of sanitizer in individual packets was stored on a shelf next to a rolling pin and other utensils. COS, moved.
- KDA_7_20111B - --In the kitchen a bottle of disinfectant and a bottle of contact solution was stored on top of the MT. COS, moved. In the wait station a bottle of hand sanitizer, lotion, and bleach wipes were stored on the counter over boxes of condiments. COS, moved.
