KDA_7_20111A - --Near the mop sink in the dry storage room, two spray bottles of sanitizer and Murphy's Oil Soap are hanging on a wire shelving unit, with the nozzles pointed towards an open box holding single-service food containers, and pizza boxes. A spray bottle of degreaser is on a shelf between packaged salsa and pizza sauce. A spray bottle of RTU glass cleaner is on a shelf beside various flavors of slushie drink mixes. COS - employees removed chemicals and placed in proper storage area near 3-vat dish sink. COS - kitchen manager knows that this was opened on Friday, 5 days ago. Date-marked properly. KDA_2_30114H - --Kitchen manager puts on a hairnet, then dons gloves to work with food, with no handwash after touching hair, before putting on gloves. COS - educated, then she washed her hands KDA_3_10111 - --Five bags of cut lettuce salad have brown, slimy portions of lettuce inside the bags. Three packages of strawberries are labeled as 16 ounces, but only hold 12, 14.8, & 14.3 ounces each. In addition, 3 of the packages have molded strawberries inside. COS by VD KDA_3_30211A1b - --In the WIC, a basket is holding 2 cartons of raw shell eggs, and is on top of another basket holding Lunchables RTE meals and containers of yogurt. COS - rearranged properly, with raw eggs on the bottom. KDA_3_50116A2 - --In the open-front case, burritos and meat sandwiches are 43F to 54F. (Bomb burrito 53F, ham & cheese sandwich & egg salad sandwich 52F, beef burrito 54F, sausage biscuit 47F). COS - removed from case by kitchen manager, and segregated for returns (42 sandwiches and 33 burritos) In the RIC under the roller grill, hot dogs are 53F, brats 53F, and tornados 53F. Temperature logs in the establishment show a history since this past Friday (5 days ago) of this cooler being over 45F ambient air temp. 3 out of 5 days, and up to 60F on Saturday, with no corrective action taken by the store. COS - removed and logged out for Returns (42 hot dogs, 36 taquito/tornados) In the open-front retail cooler - cut lettuce salad is 46F. KDA_3_50118A3 - --In the RIC under the roller grills, there are 7 containers of hot dogs, brats, and tornados - all of them are dated for 8 days instead of 7 days (11-9 to 11-16). COS - redated for 7 days (to 11-15) KDA_4_60211E4 - --There is a build up of mold inside the soda fountain nozzles, and around the ice chutes. KDA_4_90311A - --In the kitchen area, a cardboard box holding single-service plastic food trays is stored directly on the floor. KDA_6_50112A - --There is a build up of straw wrappers and other trash down beside the ice machine, which is visible from the back side in the warewashing room.h KDA_6_50116 - --A mop is left in a bucket of soiled water behind the front counter - not cleaned and hung up to air-dry.
