KDA_3_50116A2 - --Rehydrated rice noodles 48F/45F were in 5 gallons plastic buckets in front of the cooking stove. PIC stated the rice was taking out of walk in cooler at 11:00AM. COS, rice placed in the walk in cooler.

Dish machine tested 200 part per million chlorine. --Raw fish was thawing in cold running water in the 3 compartment sink.
