COS, hot dogs, and ham discarded. Raw fish was completely thawed in ROP. PIC in charged stated the raw fish had been thawed for more than 24 hours. COS, raw fish discarded. Mold was on peppers that were in the reach cooler. COS, pepper discarded.
- KDA_3_30412B - --Single-serve container without handles were stored in containers of sugars.
- KDA_3_50118A2 - --Opened package of deli ham, bologna, deli turkey, and hog head cheese that were in the display case did not have opened package dates on the food items. PIC the items were opened on 11/11/15. COS, items dated.
- KDA_4_60111A - --Dried food debris was on the blade of knife that was stored in a drawer in the meat department. COS, removed for cleaning and sanitizing.
- KDA_4_60111B - --Cooking pots and baking trays were encrusted with grease deposits and stored as clean.
- KDA_5_20511B - --Food contact meat cutting blades were stored in the handsink in the meat department. COS, items removed.
- KDA_5_20515B - --A handsink in the food prep area, and 2 handsinks in the meat department leaks water from the drain pipes below the sinks.
- KDA_6_30112 - --No paper towels were at the handsink in the bakery department. COS, paper towels provided.
- KDA_6_50111 - --Broken and missing ceramic tiles were throughout the food prep area and meat department.
- KDA_6_501111A - --10 live drain flies were in the bakery area. Drain flies were resting on food storage racks, and baking trays. No pest control invoice provided.
- KDA_6_501111B4 - --The large storage area filled with old wooden pallets, non-working equipment, old boxes, and other miscellaneous items for pest harborage.
- KDA_6_501112 - --A dead roach was on the floor next to the prepackage dried peppers in the retail area of the establishment.
- KDA_6_50112A - --Dried food debris were under the steam table, walls in the food prep areas, storage area, and along the shelves in the retail store area.
- KDA_7_10211 - --White buckets in the meat department, and in the bakery department were filled with a cloudy liquid. PIC stated the bucket container bleach water, and soap.
- KDA_7_20111A - --A spray bottle of glass cleaner was hanging on storage rack next to deli tissue paper.
- KDA_7_20612 - --A green rodent bait packet was under a retail store shelf without being inside a covered or tamper resistance bait station.
- KDA_7_30111B - --A big of instant light charcoal was stored on top of containers of dried noodles in the back storage area.
