- KDA_4_60111A - --The ice bucket is a white bucket with a sticker label on the outside for pickles. The owner verified this was originally a pickle bucket that he is now using as an ice bucket. Let him know that this sticker residue needs to be removed. COS - he removed the ice bucket so that it cannot be used. He is replacing the bucket.
- KDA_5_20411A - --There is no hand washing sink in the front bar area. The owner stated he uses one of the vats on the 3 vat sink as needed. Corrected on Site (COS) - a variance is being completed and submitted today for this process. The owner has very little dish washing to do as they only serve liquor and beer. His dish washing is limited to drinking glasses. A variance is being completed to use the far right vat for hand washing, the middle vat for washing with hot soapy water, rinsing under the faucet, and sanitizing in the final vat.
- KDA_5_40211 - --The ice machine drain line is directly plumbed to the sewer line. The line was traced to where it disappears into the floor and down into the basement. There is no break in the line. The owner is scheduling a plumber to have this fixed.
- KDA_6_20215A3 - --There is a 1/2 to 3/4 inch gap at the bottom threshold of the front door.
- KDA_6_30114 - --There are no hand washing signs posted at the hand sink for the bar area or the hand sink for the bathrooms. COS - provided with signs to be posted.
- KDA_7_20111A - --In the gray cabinet near the restrooms there are bottles of hand soap on a shelf directly next to a box of glasses. COS - the owner rearranged.
- KDA_7_20111B - --In the gray cabinet near the restrooms there are bottles of air freshener, floor cleaner, glass cleaner, and soap being stored directly on top of/above boxes of drinking glasses. COS - the owner rearranged.
- KDA_Defacto - --Cooler in Bar Area: Half & Half - 39.4 F --Establishment is open and operating during licensing inspection. Only liquor and beer are sold. There is food on site that the owner identified as his personal use. --Ownership verified. --Please note - any dairy products that are on site to be used in drinks must be date marked. Date Marking Requirements: Once the container is opened, if it is not completely used that same day it must be dated. You may write on the container what date it was opened. It may be held for 7 days. The day it is opened is day 1, with 6 days remaining for it to be used in. Example: If you opened your Half & Half today, 11/24/15, it could be used through 11/30/15. --Please note, all chemicals must be stored separately of food, drinks, paper towels, equipment food may be stored in, consumed out of, etc. It is a violation to have any chemical items stored next to, or above, such items. --The owner provided chloro tablets as the sanitizer for the dishes, along with chlorine test strips. --There is hand soap to the right of the 3 vat sink and paper towels to the left of the 3 vat sink. --There is no mop sink. They typically do not mop as there is little need. They use a Swifter mop most of the time. If there are spills requiring further mopping, the owner has a cleaning company that mops as needed. He uses a bucket to dump water from his sink into the mop bucket. The mop water will be dumped down the toilet. The bucket and mop head would be soaked in chemicals for cleaning/laundered as needed.
