- KDA_2_30114G - --While wearing gloves, employee cracked raw shell eggs into a pitcher, then immediately wisp/blended eggs then handled RTE cubed ham. COS: educated, hands washed, ham cooked on flattop grill.
- KDA_3_50118A1 - --In the WIC, of approximately 40-50 dated PHFs, one opened package of deli chicken breast was dated 10/19. COS: discarded.
- KDA_4_50111B - --The seals on the prep table were cracked and peeling away from doors. One was held onto door by duct tape.
- KDA_4_60111A - --Six metal pans stored as clean soiled with dried food debris and rust. Blade of counter mounted can opener was soiled with dried food debris. Knife stored as clean on magnetic strip was soiled with dried food debris. COS-removed for washing. The blade and housing of the meat slicer were soiled with dried food debris.
- KDA_4_90311B - --Three metal pans stored as clean were stacked/nested wet, not in self-draining position. COS: removed for washing.
- KDA_7_10211 - --A bucket of liquid on floor, identified by the PIC as sanitizer, was not labeled. COS: labeled.
