- KDA_3_30414B1 - --The cloth bucket on the east side was at 0PPM Quat. Employee said buckets were made at 8:30AM. COS- Quat solution was re-made to 200PPM.
- KDA_3_50116A2 - --On the salad bar sitting in ice: cottage cheese 53.8F, shredded cheese 63F, diced commercially processed turkey 58.7F, diced commercially processed ham 58.5F and sliced hard boiled eggs 63.1F. Person in charge said these items had been out since 10:15AM. They are pulled off and put away at 11:45PM. COS-More ice was added to salad bar.
- KDA_3_50118A3 - --In the walk in cooler, there was deli turkey opened and wrapped in plastic wrap that had an open date of 9/28/15. There was also an open package of kale salad mix(cut cabbage and lettuce in it) with an open date of 9/28/15. COS-these items were discarded.
- KDA_4_20211A2 - --In the rubber spatula drawer on the long prep table on the west side, there were three rubber spatulas that were missing pieces out of the food contact surface. COS-items were discarded.
- KDA_4_60111A - --On a shelf in the ware washing area, there was an empty container drying with blue tape labeling 5/11/15. In the storage room, there were three plastic containers with sticker residue on them. COS-these items were placed at ware washing machine.
- KDA_4_90411A - --In the following areas there were serving utensils that had handles not all going the same way, so food contact surfaces were coming in contact with hands: long prep tables on the east and west side, in the stack drawers. Also the drawers on the east wall. There are also serving utensils on a shelf near the three compartment sink that have handles that will come in contact with hands.
- KDA_6_50111 - --Above the stove on the west side, one ceiling tile is missing. In the chemical storage room, there are several ceiling tile missing. There are three stained in that area also.
- KDA_6_50112A - --In the cooking area, there are dusty tile through out.

On the steam table pork loin 142.9F. --On the stove top lima beans and corn 185.7F. --On the stove top, lima beans and corn 185.7F. In the oven, diced potatoes 174.F.
