--Tray of toasted cheeses sandwiches placed on top of serving line counter for student self service not protected from student cough and sneeze contamination. Tray of sandwiches removed to behind sneeze guard.

--Dishmachine pressure gauge still not working, not registering 15-25 psi during final rinse.
- Equipment Openings, Closures & Deflectors (46.582)
--Hot box door gasket still loose, not attached on sandwich bar side of serving line area. Ice buildup still an issue beneath front evaporation unit within walk-in freezer, trays of ice collecting on shelf below unit. Shelving unit cannot be used for food storage.
- Maintenance & Operation of Mechanical Warewashing Equipment (46.675(a)-(c))
--Motor switch on high temp dishmachine is not operating properly, unit kicks off after switch pushed to start / on, has to be reset (sometimes two or three times) to start carriage movement / machine operation. FSD to call for service immediately before unit won't run at all.
- Plumbing Facilities in Good Repair, Cross Connection Prohibited, Scheduled Inspections, Cleaning Water Reservoirs (46.825(b)-(e))
--Sinks faucets still leak / won't shut off on prep area two basin and three basin sinks. Far right sink drain of three bowl sink leaks, hotel pan beneath to catch leaking water. Staff not using the garbage disposal on two bowl sink counter due to odd noise when operated. Service / repair as needed, as discussed, keep disposal flushed and clean, even if not in use.
- Repair & Cleaning Various Premises, Structures, Attachments & Fixtures (46.981(a)-(c)(g)(n))
--Inoperative, unused equipment in kitchen and hallway to kitchen still to be removed.
- Controlling Pests, Removing Dead or Trapped Birds, Insects, Rodents and Pests (46.981(k)(l))
--Pest control measures taken and in place, thank you. Staff related that no new activity has been noted in stockroom area, no evidence of activity found elsewhere in kitchen. As shown to Kitchen Manager, there are still some scattered droppings along wall corners in stockroom, possibly left behind when cleaning with a broom. As discussed, obtain a shop vac and remove / clean ALL droppings so as to monitor for any new activity in the future. Address immediately.
