    Shelves storing bottles next to office and in walk in cooler were found to be made of raw wood.  Seal or Paint to ensure a smooth, durable, non absorbent, easy to clean surface.

All frozen pizza was frozen during this inspection.

As of January 1, 2016, all Food Handlers (bar tenders and/or serving staff included) are required to take a 2-hour online training course. All courses available are on the Illinois Department of Public Health website, under Food, Drugs and Dairies. Once this class is completed, print the certificate out for any inspections in 2016.

All plumbing with the ice machine appears to be correctly installed and functioning properly during this inspection. All plumbing shall be installed and sized in accordance with the Illinois State Plumbing Code.

This is a recheck inspection for the establishment for a previous inspection done. It is important to note that other non-critical items were fixed as requested at the last inspection. During this inspection the items noted as being corrected include the wall in the mop sink room (it no longer has a hole and is now a smooth and easily cleanable surface), the sealing/painting of table at front of bar entrance was completed as well. The owner stated that the kitchen will not be used for food preparation or service.  In the future, the facility may begin to store and serve pre-packaged frozen pizzas.  Only pre-packaged, frozen pizzas will be allowed for serving under the current Category III license.

From Violation #30: The only issue now is the drainage of the ice machine from the condensate drainage, this is a minor violation as noted by the code: Drains from commercial dishwashing machines, dishwashing sinks, pot washing sinks, pre-rinse sinks, silverware sinks, bar sinks, soda fountain sinks, vegetable sinks, potato peelers, ice machines, steam tables, steam cookers, and other similar fixtures shall be indirectly connected in compliance with 77 Ill. Adm. Code 890.1410(a).  This will be noted as a violation #29 in future inspections if not corrected. The major violation has been corrected. A licensed plumber should be contacted in regards to being properly plumbed. Should there be any backflow from the condensation pipe, the ice machine must be completely drained, cleaned and sanitized. The City plumber will be contacted for further evaluation.
  • Critical: Water source safe, hot, and cooled under pressure
    During previous inspection, men's left side hand washing sink did not have hot running water or proper water pressure for handwashing.  Valve beneath handwashing sink was not turned on.  After turning on valve for water, hot and cold water were available under pressure.
  • Critical: Cross-connections, back-siphonage, back-flow prevented
    During previous inspection it was determined that no air gap or backflow device was present for ice machine drain.  Ice machine drain was sitting directly in pvc pipe.  Corrected by installing backflow prevention device for ice machine into the water system, this was noted by the picture where there is a back flow device.   SEE NOTES SECTION AS THIS CONTINUES

HACCP:  Hand washing sink accessibility.  Make sure all hand sinks are accessible for hand washing at all times and that nothing blocks accessiblity.  

Chlorine sanitizer is used.  No sanitizer was made at time of inspection.

Only pre-packaged foods (chips) are on the premises.
  • Critical: Water source safe, hot, and cooled under pressure
    Left side sink in men's restroom does not have hot water or pressure for cold water.
    Water under pressure at the required temperature shall be provided to all fixtures and equipment that use water.
    Lavatories shall be provided with hot and cold water tempered by means of a mixing valve or combination faucet.  A recheck inspection will be done on or after 9/24/13.  A recheck fee will be charged.

    Drain hose from ice machine does not have air gap or backflow prevention device.
    There shall be no cross connections between the safe-water supply and any unsafe or questionable water supply, or any source of pollution through which the safe water supply might become contaminated are permitted.  A recheck inspection will be done on or after 9/24/13.  A recheck fee will be charged.
  • Walls, ceilings, and attached equipment, properly constructed, clean
    1.  Mop sink wall in disrepair and exposed.  2.  Wood surface behind bar needs painted or sealed.  3.  Unsealed wood on bar table at front entrance.
    Walls and ceilings, including doors, windows, and similar enclosures shall be maintained in good repair.
    Walls, including non-supporting partitions, wall coverings and ceilings of walk-in refrigerating units, food-preparation areas, equipment-washing and utensil-washing areas, toilet rooms and vestibules shall be light-colored, smooth, non-absorbent and easily cleanable.

This establishment has been given final approval to operate under the guidelines of the Chapter 10, Food Safety, of the Peoria County Code.

This approval is for the bar only, nothing will be stored, or used in the kitchen area except for the walk in cooler which has a seperate hallway enterance.

If food will be served out of the kitchen, another plan review application must be submitted for approval.

    Several areas have unsealed wood including shelving on rolling cart, shelves on far side of bar near ice machine room, and back underside of bar.
    Seal wood.

    As of right now, only cans and bottles will be served from the left side of the bar near the ice machine room.  If ANY food prep or drink mixing will take place on the left side of bar, a handsink must be installed on that end.
  • Restrooms with self-closing doors, fixtures operate properly, facility clean, supplied with hand soap, disposable towels or hand drying devices, tissue
    Provide soap and paper towels at all handsinks.
  • Walls, ceilings, and attached equipment, properly constructed, clean
    Walls near mop sink are in major disrepair.

    No light is provided in the walk in cooler.
