  • 229.165 (d) (1) (B)free of breaks, cracks. Observed many spatulas that were not smooth and easy cleanable. Ensure all utensils are free of breaks, cracks, chips, inclusions, pits and similar imperfections.

  • Air gap between water supply inlet and plumbing fixture is not present. THE SPRAY HOSE TOUCHES THE BASE OF THE SINK, SPRING TO BE REPLACED IN ORDER TO HANG PROPERLY.
  • Chlorine concentration for chemical sanitizer in mechanical ware washing equipment not in line with PH/concentration chart. Use any chemicals for sanitization according to manufactures directions. DISH WASHER IS NOT HOLDING THE SANITIZER, INSPECTOR FOUND 0PPM CHLORINE DURING SANITIZER CYCLE. MUST CALL COMPANY OUT TO RE-CALIBRATE MACHINE. DISCONTINUE USE OF DISHWASHER UNTIL CORRECTED. YOU HAVE THE OPTION OF USING THE 2 COMPARTMENT SINK AVAILABLE ONLY IF YOU CAN DEMONSTRATE THE WASH, RINSE AND SANITIZE PROCESS PROPERLY.
  • FIX ICE MACHINE TO PREVENT WATER LEAKING ALL OVER THE FLOOR. REMOVE ALL TOWELS AND REPAIR MACHINE AND ELIMINATE WATER COLLECTING ON FLOOR.

NOTE: COndemned and voluntarily destroyed (IAW city ordinance) approx. 60 lbs of TIme-temperature abused productmilk,raw proteins,cut produce,etc.) 50 F for more than 4 hrs. NOTE: Not authorized to use WIC for storage of PHF until unit is inspected by the Health Department to assure unit maintains 41F or less.
  • Cleaning & sanitizing of equipment and wares not executed to protect from cross contamination. Protect food from cross-contamination by cleaning and sanitizing equipment and utensils. NOTE: Assure sanitizing containers which are stored on the floor, are not not placed on food contact surfaces. NOTE: Wash,rinse and sanitize work tables and store containers in a manner in which food-food contact equipment/supplies are not contaminated.
  • 229.166 (c) (3) hot water sufficient(hand sinks/mop sink(110 F)). Hand sink has no hot water. Hot water at hand sink(93 F) is less than 100 degrees. Hand sink must provide hot water of at least 100 degrees through the use of a mixing valve or combination faucet.
  • 229.166 (f) (4) air gap 2x. Air gap on ice machine is not present. Provide for an air gap with 1 ? inches or two times the diameter of the pipe.
  • 229.165 (g) (1) equipment not sufficent. Cooler(WIC) not cold enough to hold establishments cold PHF. Provide adequate refrigeration.
  • 229.167 (e) (3) (A) no towels. There are no paper towels at the hand washing sink(Employee restroom). Supply the handwash station with individual disposable towels.
  • NOTE: (IAW the COSA Municipal code chap. #13) during the establishment?s hours of operation a Certified Food Manager with a valid Health Department Certificate, both must be present. Once a valid Food Managers Certificate is obtained, proceed to 1901 South Alamo (DBSC) Health Dept. desk with the certificate to purchase ($15.45) a Metro Health certificate.
