- Observed shelving in kitchen that is beginning to rust. Seal or replace.
- Observed debris accumulation on the following items/surfaces: Soda gun holders in bar, inside of ice machine, and inside and outside of all cooling units especially around the seals. Wash, rinse, and sanitize these items/surfaces and ensure routine cleaning.
- Observed chipped and peeling floor throughout kitchen area where the ice machine divides the kitchen. Seal.
- Observed debris accumulation on floors in the dry storage area. Clean and ensure routine cleaning.
- Observed holes in the wall next to the shelving where dishes are stored. Repair.
- There are several ceiling lights out throughout the kitchen. Replace.
- Staff at facility is not in compliance with food handler requirements. Ensure all staff who currently do not have a FSSMC license obtain their food handlers certification by 11/30/15.

- Observed buildup on floors in walk in cooler and in dry storage area. Clean and ensure routine cleaning.
- Floors throughout kitchen are chipping. Seal. Observed coving beneath walk in cooler coming off wall. Repair or replace.
- Observed hole in wall by refrigerators. Repair. Observed walls and door in kitchen unsealed. Seal. Wood unsealed on paper towel holder in bar. Seal.
- Observed debris accumulation on walls near 3 compartment sink. Clean.
- Observed several lights throughout kitchen that were not operable. Repair so that all lights are working.
- Observed non functional equipment in kitchen. Equipment must be operational or removed from facility.

- The water heater is very rusted and appears to have buildup on it. There is no dust cover on the reach in cooler and freezer in the kitchen. Any unsealed wood needs to be sealed. For example, in the entry way to the kitchen there is wood.
- There is food buildup on the drip trays in the pizza ovens, microwave, toaster, shelving.
- Observed gap on the bottom right of the door to outside in the back of the kitchen. Door must be repaired so no daylight is visible. This needs to be addressed by 12-19-14. Contact Katie once the door has been repaired.
- The ceiling is damaged above the water heater in the kitchen.
- Found multiple tools/articles not necessary to daily operations in the facility. Remove unnecessary articles from the facility.
