  • Observed pork chops in cookline cooler with a date label of 1-28. Per employee, that date was incorrect and pork chops were in cooler for a couple days. Food was voluntarily discarded by employee. Ensure all ready to eat potentially hazardous food is correctly date labeled and used by or discarded 7 days after first day it's open or prepared.
  • Observed cigarette butts upstairs equipment storage room and a strong smell of cigarette smoke in upstairs restroom. Per state code, smoking is not allowed inside of public facilities.
  • Observed rust buildup on shelves in left walk in cooler and white walk in freezer. Also observed damaged door seals of walk in cooler, white walk in freezer, and reach in coolers and freezers in cookline. Repair or replace damaged shelves and door seals.
  • Observed food and grease buildup on front and insides of 3 cookline microwaves. Routinely wash, rinse, and sanitize these surfaces.
  • Observed debris buildup on upstairs dry storage shelves, white walk in freezer shelves, and door seals at 2-door reach in freezer. Routinely clean these surfaces.
  • The drain pipe under the 3-compartment sink and dish room hand sink was broken and leaking on the floor. The 3-compartment sink faucet could not be shut off all the way and was also leaking. Contact a licensed plumber and repair or replace all damaged plumbing.
  • Observed stack of dish trays being stored on floors in front of dish room handsink. Discussed keeping the handsink accessible with employee; employee removed trays from area.
  • The floor between the cookline stove and freezer had buildup of grease and debris. Also the floors throughout the facility; particularly upstairs dry storage area and white walk in freezer, had build up of debris. Routinely clean the floors.
  • Observed cracked or missing floor tiles in the following areas; upstairs food storage area, walk in coolers, and white walk in freezer. Replace or install missing floor tiles. Also re-seal floor coving to wall, just outside walk in cooler.
  • Repair badly damaged wall next to 2 door cooler across coffee station. Caulk behind the dish room 3-compartment sink and dish room hand sink where it attaches to the wall. Also routinely clean vents in walk in coolers to prevent dust buildup.

  • White gravy, baked potatoes, mashed potatoes, and green beans were 46-47F in the West walk in cooler and were discarded. These items were cooked and cooled yesterday, per label. Other foods in this unit was at 42F-43F and moved into the other walk in cooler as a precaution. Cole slaw and sour cream in the bottom of the salad prep cooler were 49F and 46F and were discarded. Food in the top of this unit was 41F or below. Maintain cold food at 41F or below. Repairman was contacted during the inspection. Do not store potentially hazardous foods in the bottom of this unit until it is repaired and can maintain food at 41F or below. Call TCHD for a re-inspection when repaired.
  • Observed cigarette ashes in the hand sink in the upstairs staff bath room.and a strong smell of smoke in this bath room. Per state code, smoking is not allowed inside of public facilities.
  • Repaint or replace the rusty shelves in the west walk in cooler.
  • Repaint the chipped and rusty shelf across from the walk in freezer with the sheet pans stored on it. Replace the rusted out shelf/table in the dish room. Repair or replace the broken door seal to the salad prep cooler.
  • The grill, stove, and fryers had a build up of grease and debris. Clean and sanitize them.
  • The insides of the reach in coolers on the grill line and shelves in the west walk in cooler had a build up of debris. Clean and sanitize them.
  • The drain pipe to the 3-compartment sink and dish room hand sink was broken and leaking on the floor. The east corner of the 3-compartment sink had a small hole in it and was dripping onto the floor. The 3-compartment sink faucet could not be shut off all the way. The drain pipe under the hand sink by the walk in coolers was leaking onto the floor. Repair or replace all damaged plumbing.
  • Replace missing floor tiles in the dish room, walk in freezer, and west walk in cooler. Grout or fill the gap in the floor tiles where there is a step up into the dish room.
  • The floor had a build up of grease between the stove and freezer in the kitchen. The upstairs dry storage area floor had a build up of debris. Clean the floors.
  • Repair the holes in the walls in the west walk in cooler. Replace the missing wall tiles in the south wall by the door in the kitchen that leads outside. Caulk behind the dish room hand sink where it attaches to the wall. Paint the bare wood on the south wall in the kitchen and under the hand sink by the walk in coolers. Repaint the rusty bottom area under the door to the west walk in cooler.

  • Paint bare wood above kitchen hand sink. Replace rusty shelf in the dish room. Remove tape on wire shelf outside office. Repaint chipped shelves under prep tables outside walk in freezer.
  • Clean walk in cooler shelves to remove dut and debris.
  • The hand sink by walk in coolers was slow draining, and leaking from the drain pipe. Water was coming up inside the floor drain when the faucet was running at this hand sink. The drain pipe from this hand sink was leaking onto the floor. The drain pipe under the hand sink in the dish room was still raining onto the floor. Repair all these items and provide documentation from a licensed plumber for the repair to the floor drain outside the walk in coolers to Kim at TCHD by 8/26/15.
  • Replace damaged and missing floor tiles in the walk in coolers and freezer, on stairs, and in the kitchen.
  • Repaint chipped walls in the walk in coolers. Replace missing wall tiles under prep table by back door. Repair the remaining hole in the wall under the waitstaff counter.

  • Observed black growth on pickle container in walk-in cooler. Food items must be kept in sound, clean condition during storage. Pickles were voluntarily discarded by employee.
  • Observed batter mix and cinnamon in bulk food containers without labeling. Bulk food removed from original container must be labeled for proper identification. Label bulk food containers in facility.
  • Observed sausage patties, ground beef, gravy in walk-in cooler without date labeling. All potentially hazardous food must be date labeled during storage. Employee voluntarily discarded all items without date labeling. Discuss proper date labeling with all employees and document training.
  • Observed masking tape used as repair on food cart. Masking tape is not an approved material for repair in food service. Repair cart with approved material. Observed prep table and table in service area with chipping paint. Food contact surfaces must be smooth and easily cleanable. Seal, paint, or replace surfaces.
  • Observed table in dish room is rusted and damaged. Non-food contact surfaces must be in good repair. Repair or replace.
  • Sanitizer in bucket located in service area had concentration of less than 50 ppm. Chlorine sanitizer in buckets must be held between 100-200 ppm. Waitress corrected immediately.
  • Observed shelving with build up of debris in facility, especially in walk-in coolers. All shelves holding food must be clean. Clean shelves. Observed containers holding clean equipment were visibly soiled. All containers holding clean equipment or utensils are considered food contact surfaces. All equipment in dirty containers must be washed, rinsed, and sanitized before use. Dirty containers must be cleaned and sanitized. Employee removed all equipment from shelving.
  • Observed vents dirty throughout facility, especially in main kitchen and bathrooms. Clean routinely. Observed visibly soiled shelving in chemical storage room. Clean routinely.
  • Observed leak coming from rubber gasket under handwashing sink in dish washing area. All plumbing must be maintained in good repair. Repair. Observed sewage back up from drain located near hand sink in main kitchen. Only floor drain in facility that is experiencing back ups. Must have drain repaired by licensed plumbing contractor and documented.
  • Observed no hand towels at hand sinks in kitchen and service area. All hand washing sinks must have single service towels available for proper hand drying. Employee replaced immediately.
  • Observed damaged floors that affect cleanability throughout kitchen, service area, and walk in coolers. Repair.
  • Observed build up of debris on floors under equipment, in corners, in coolers, and throughout kitchen. Clean floors.
  • Observed walls and ceilings in walk-in coolers, kitchen, and dish area had build up of debris; especially behind all equipment in kitchen. Clean walls and ceilings.
  • Observed missing vent cover upstairs on ceiling. Replace vent cover. Observed missing wall tiles underneath counter in service area. Replace tiles. Observed hole in ceiling above cook line. Repair hole. Observed paint chipping on walls in kitchen and walk-in cooler. Repaint or reseal. Vent cover missing bathroom. Replace vent cover.
