Critical procedue inspection focusing on handwashing, glove use, handwashing, & temperature logs. No violations observed. Records for Feb. and March checked for thermometer calibration, cooking temperatures, hot holding temperature, and equipment temps. Dishwashing log also being kept.
On your hot holding temperature chart, write the name of the unit at the top of the page, just to eliminate any confusion as to what is being monitored.

If unable to repair the @EQUIPMENT@, replace it with one that meets the specifications of Part IV, Article 1 and 2 of this chapter. The lid has been ordered, replace when it arrives.

Comments:
Extermination efforts have been increased. There were no signs of droppings in the kitchen, nor were any paths or burrows noted outside. The situation has been discussed with the principle. Continue to monitor and contact the exterminator if the problem recurs.
