• 1750 - In the large walk-in refrigerator, there was tuna macaroni salad being stored in a commercial container with a commercial label on it that says "sour cream." There was a pasta salad being stored in a commercial container with a commercial label on it that says "oysters."
  These containers are intended for a "single-use only," by the food suppliers. Only the foods which originally came in these containers may be stored in these containers. Use "tupperware" or some other easily cleanable, reusable container for these storage applications.
• 3170 - Repeat There is an area of the ceiling in the kitchen, above the exhaust hood and above the deep fryer units, that has rough surfaces and is no longer easily cleanable.
  We require that all areas of the ceiling (and the walls and the floor) be maintained so that they are smooth and easily cleanable. This area of the litchen ceiling needs to be repaired prior to renewal of the permit, on, or before 31-Oct-2009.
• 2890 - Repeat Protective shielding is missing for the two ceiling fluorescent tubes above the mechanical dishwashing area.
  We require that all lighting in kitchen and food prep areas and food storage areas be shielded.

January 08, 2009 (Routine)

• 1190 - There is no "hanging" or "display" thermometer in the white GE refrigerator (near the grill), and there is no "hanging" or "display" thermometer in the white westinghouse refrigerator (on the side of the bar area-next to the kitchen).
  We require that all refrigerators (and all freezers) have a "display" or "hanging" thermometer in them at all times. Thermometers need to be put in place in these refrigerators.
• 1530 - The (bleach-type) paper test strip kit, that we require to be on site at all times, for testing sanitizer level associated with the final step (the sanitizing step) of the mechanical dishwasher, could not be located.
  Needs to be purchased as soon as possible, and kept on-site. Available chlorine level for bleach type sanitizers always needs to be 50 to 100 ppm, as measured by the proper test strip.

June 18, 2008 (Routine)

• 2890 - The celing light bulbs in front of the mechanical dishwasher are missing the required safety shielding.
  We require that all light fixtures in kitchen and food prep areas be shielded.
• 3170 - There is a small, non-cleanable rough area surrounding a "crack" in the ceiling, in the kitchen, in front of the deep fryers. A window screen is missing for the kitchen window near the front of the kitchen.
  The crack in the ceiling, and the surrounding area of the ceiling needs to be repaired. We require that all areas of the kitchen ceiling be smooth, and easily ceanable. The missing kitchen window screen needs to be put back in place, before a fly problem develops in this kitchen.

December 06, 2007 (Routine)

• 1080 - Corrected During Inspection There were two metal-banded pastry brushes observed in the kitchen.
  We do not allow metal-banded pastry brushes in food establishmements, because the areas between the metal band and the brush are not cleanable, at the bacterial level, and the metal bands can rust. The metal banded brushes were immediately removed from the restaurant by the management. This corrected the violation.
• 2890 - Two fluorescent ceing bulbs, above the mechanical dishwasher area, in the kitchen are mising the required safety shielding.
  We require that all lighting in kitchen and food storage areas be shielded. Safety shielding needs to be put in place for these two ceiling bulbs.

May 31, 2007 (Routine)

• 1570 - Corrected During Inspection There are cracks in the plastic ice scoop, stored to the left of the big ice machine (located in the back part of the kitchen) and duct tape has been used to attempt to repair this ice scoop.
  This ice scoop is no longer cleanable at the bacterial level. And duct tape may not be used for repairs because the duct tape introduces additional rough surfaces. This ice scoop was immediately discarded by the management. This corrected the violation.
• 3170 - The ceiling has a rough area which is no longer cleanable, above the big ice machine, across from the front of the larger walk-in refrigerator. Also, the wooden support bracing, near the finished plate area, in the center of the main kitchen room, needs to be painted, or sealed with a wood sealant.
  We require that the ceilings in all kitchen and food storage areas be smooth and cleanable and impermeable to moisture. All wood needs to be sealed or painted, so that it is easily cleanable, and impermeable to moisture. These repairs need to be made prior to renewal of the permit, on or before 31-Oct-2007.

January 18, 2007 (Routine)

• 0450 - Corrected During Inspection Critical A foodservice worker was touching ready-to-eat foods (chopping green peppers, in the sald prep area) with their bare hands.
  Bare hand contact with ready-to-eat foods is not allowed. Gloves, tongs, and spoons must be used. This was immediately corrected by the establishment management.
• 0470 - Corrected During Inspection Critical There was a pan containing raw fish fillets being stored above a covered pan of cooked ribs, in the smaller walk-in refrigerator. There was a bowl of raw shrimp being stored on a shelf above whole lemons, in the larger walk-in refrigerator.
  Raw meats and raw seafood must always be stored below all precooked and ready-to-eat foods. This violation was immediately corrected by the management.
• 1570 - The tyler 2-door reach-in refrigerator has a rust build-up, inside, in the bottom.
  This refrigerator is not in use at this time. The interior of this refrigerator needs to be refurbished, before the refrigerator may be again used for food storage.
• 1770 C - There is visible .black mold on the walls in the smaller walk-in refrigerator. There is visible debris, and water, in the bottom of the ice scoop storage container, associated with the big ice machine.
  The interior of this walk-in refrigerator needs to thoroughly cleaned, and repainted, as soon as possible. The ice scoops and the ice scoop storage container need to be washed, rinsed, and sanitized, and kept on a more frquent cleaning schedule.

July 06, 2006 (Routine)

• 1750 - Repeat There are plastic "oysters"buckets, and plastic buckets with commercial writing on the side of them which says "amish salad," that are being re-used for storage of other foods. There is a paper label (and glue below) on the sides of the "amish salad" plastic buckets.
  These plastic buckets, and lids, are intended for a "single-use" only by the manufacturer. These containers do not stand up to repeated washing procedures. Only the foods that originally came in these buckets (and lids) may be stored in these buckets. Reuse of the buckets with glue and paper labels on the side of them is "risky" because they cannot be cleaned at the bacterial level. The paper and glue introduce places wher bacteria may "hide." These buckets need to be replaced, as soon as possible, with tupperware, or some other more durable type of food container.
• 3180 - There is a build-up of dust around the fluorescent ceiling light fixtures in the kitchen.
  This dust needs to be removed, and these areas need to be cleaned more frequently.

December 21, 2005 (Routine)

• 0540 - Corrected During Inspection Critical There was a container of raw chicken wings above covered containers of shrimp pasta salad, on the shelf unit in the smaller walk-in refrigerator.
  Raw meats and raw seafoods alway need to be stored below the ready-to-eat foods. This violation was immediately corrected by the management. The container of raw chicken wings was placed on the bootom shelf, below the ready-to-eat foods.
• 1080 - There is duct tape wrapped around the handle of the sprayer unit in front of the mechanical dishwasher.
  The duct tape is not allowed because it has rough surfaces. That introduces places where bacteria can hide. The sprayer head must be smooth and easily cleanable at all times. Repair the sprayer head if necessary. Remove the duct tape.
• 1750 - There are seafood and pasta salads being stored in white plastic buckets that originally contained cottage cheese or sour cream. These white plastic buckets and lids are specified to be used for a "single-use only," by the manufacturer.
  Only the food which originally came in these containers may be stored in these containers. Replace these containers with tupperware or more durable containers which can stand up to repeated washings.
• 3170 - There are floor tiles missing in the kitchen, exposing rough surfaces which are no longer cleanable at the bacterial level. There is rust on the inside of the door , and on a wall inside the walk-in refrigerator.
  We require that the kitchen floors be smooth and cleanable at all times. Floor tiles need to be put in place where they are missing. We require that the walls, and the surface of the door, inside the walk-in refrigerator, be smooth and easily cleanable. The areas with rust need to be sanded and repainted.

June 23, 2005 (Routine)

• 0470 - Corrected During Inspection Critical Repeat There was a package of raw ground beef, and a package of raw chicken, stored above lettuce leaves, lemons, and tomatoes, in the larger walk-in refrigerator.
  Raw meats must always be stored below all ready-to eat foods, and all precooked foods. This violation was immediately corrected by the management.
• 0550 - Corrected During Inspection Repeat There were two ice scoops being stored directly on the top of the big ice machine, in the back of the kitchen area.
  These ice scoops need to be stored in a smooth, cleanable container, or on a smooth, cleanable plate or tray. Not directly on top of the ice machine.
• 1700 - Critical The mechanical dishwasher was getting no detectable sanitizer, associated with the final step of the mechanical dishwasher cycle.
  We require that the available chlorine level always be 50 to 100 ppm, associated with the final step of the mechanical dishwasher cycle. Until the mechanical dishwasher is again getting the proper level of sanitizer, we require that all items be sanitized manually, in the three compartment (dishwashing) sink, following the run through the mechanical dishwasher. We require that the establishment call us, and leave a message, as soon as the problem with the mechanical dishwasher has been corrected.
• 3080 - There is not enough light in the larger walk-in refrigerator. There is not enough light above the cutting/food prep area in the front part of the kitchen, near the two compartment prep sink.
  We require that lighting intensity be at least 10 foot candles in walk-in refrigerators. We require that lighting intensity be at least 50 foot candles, at the work surface, in cutting areas, and areas of the kitchen where a meat slicer is located.

December 22, 2004 (Routine)

• 0470 - Corrected During Inspection Critical There is a package of raw ground beef, on a tray, on a shelf, above an open case of lemons, in the larger walk-in refrigerator.
  Raw meats always need to be stored below all ready-to-eat foods. This is true, even when the ready-to-eat foods will be cooked, prior to service. This violation was immediately corrected by the owner.
• 2260 - Critical The sprayer head of the "sprayer arm assembly," between the mechanical dishwasher and the dishwashing sink, extends, when hanging freely, below the top of the sink basin, located directly below.
  We require that there be an "air gap" of one inch, or larger, between the bottom of the sprayer head (when hanging freely), and the top of the sink basin, below. This prevents the possibility of "back-flow" from this sink into the sprayer head (and possibly the public drinking water supply). The "rinse arm" needs to be shortened. The owner immediately contacted maintenance personnel to correct this problem.
• 1570 - Paint is peeling, and there are several spots of rust inside the larger walk-in refrigerator (on the door, and on the wall to the left of the door). The hot water handle at the bar three compartment sink is missing.
  The inside of the walk-in refrigerator needs to be scraped, and repainted, in areas where paint is peeling, and/or rust exists. We require that all wall surfaces inside the walk-in refrigerator be smooth, and easily cleanable, at all times. The hot water associated with the faucet, of the bar three compartment sink, where a wrench is being used to turn on and off the water, needs to have the handle replaced, as soon as possible. We require that all equipment be maintained in good repair.
• 0960 2 - The duct tape that is wrapped around the handle of the sprayer, located between the mechanical dishwasher, and the dishwashing sink, is not allowed. The cardboard and duct tape that has been used to make a "temporary" repair of the air exhaust system, in the kitchen, is not allowed.
  Only materials which are smooth, easily cleanable, and durable enough to stand up to repeated cleanings may be used to make repairs in kitchen, food preparation, and food storage areas. These two items need to be properly repaired, using appropriate materials.
• 3180 - There is a build-up of dust, in the kitchen (1) around two ceiling light fixtures in the front part of the kitchen, (2) behind and above the hobart 2-door reach-in refrigerator, and (3) around the fire supression system tank, in the kitchen.
  The dust needs to be removed. The physical facilities need to be cleaned as often as needed to keep them clean.

June 23, 2004 (Routine)

• 0550 - Two of the three ice scoops on top of the big ice machine are sitting directly on top of the ice machine.
  Ice scoops on top of the ice machine (above the door to the ice bin, and therefore not easily cleanable) need to be stored on smooth, cleanable plates or trays, or in smooth, cleanable containers.
• 0240 - Several employees in the kitchen, preparing/handling food at the time of this inspection, are not wearing the required hair covering.
  All employees in kitchen areas are required to wear a hat, hairnet, open visor, or some other type of approved hair covering, at all times. Especially important when preparing/handling food.
• 1750 - Repeat White plastic buckets, and lids, which are intended to be "single use" items, by the manufacturer, are being used to store foods, other than the food that originally came in the container. For example, in the walk-in refrigerator nearest the hot water heater, there is a white plastic bucket containing "crab salad." This white plastic bucket has "Mrs. Crocketts" impreganated on the outside of it, and "cottage cheese" impregnated on the top of it.
  Only the foods which originally came in the container may be stored in these containers. These storage containers need to be phased-out of the operation. Replace them with tupperware, or some other smooth, cleanable, durable container, which can stand up to repeated washings.
• 2890 - The light bulb in the walk-in freezer, and one of the light bulbs in the larger of the two walk-in refrigerators, are missing the required shielding.
  We require that all lighting in kitchen and food prep areas be shielded at all times.
