Violation: 3180 - The floor under the prep coolers noted in need of cleaning. All floors, walls, and ceilings must be cleaned as often as necessary to keep them clean. Cleaning of the physical facilities is an important measure in ensuring the protection and sanitary preparation of food. A regular cleaning schedule should be established and followed to maintain the facility in a clean and sanitary manner.
Comments:
Correct today. The big 4 reportable illnesses along with signs and symptoms was reviewed today. Handouts covering illness symptoms that need to be reported to management was left with manager. Food temperature were good. The quat sanitizer at 3-vat sink was 200 ppm.
September 17, 2007 (Complaint)
Comments:
The purpose of this visit was due to a complaint of an ill worker. Spoke to managment who was unaware of the complaint. Verified employee is not working at this time and is not scheduled to work untill Sept 22. Manager stated that employee will not work until they have a doctor's note showing they are well and cleared to work with food. Discussed signs and symptoms of illness and when to exclude employee.

Quat sanitizer was 200 ppm. Employee was observed wearing gloves while handling read-to-eat foods. Food temps were good. Reviewed the 4 reportable illnesses along with signs and symptoms. Clean and well maintained facility.

Violation: 2000 - Single service items observed unprotected from contamination as the coffee filters were not covered. Store single service items in its original protective packaging or in a closed container.
Comments:
Correct the above item this week. Clean facility.

Violations:
2830 - Floor and wall juncture around cabinets are not sealed. Seal wall juncture on right side of 3-vat sink. Seal floor and wall juncture in rooms where water flush cleaning methods are used.
3170 - There is a hole in the floor tile around the conduit for electrical lines. Seal floor around the pipes coming up through the floor and round the floor drain.
3670 - An application for permit has not been received by the Health Department. Submit application prior to opening for business
