The purpose of this visit is to check on the repair or replacement of the two door prep cooler. The two door prep cooler, which was malfunctioning, has been replaced with a new prep cooler. The new prep cooler is now capable of maintaining potentially hazardous foods (TCS) at 41f or below.
No violation noted during this evaluation.

  • Critical: Cold Holding of Potentially Hazardous Food at 41°F or less (corrected on site) (repeated violation)
    Observation: The following food item(s) were observed cold holding at improper temperatures using a calibrated food temperature measuring device: Observed raw hamburger 44f-45f, raw salmon 50f, raw pork 45f inside the Superior two door undercounter cooler.
    Correction: The manager relocated the above foods to the walk in refrigerator as the Superior two door undercounter cooler is not in working order. Per manager the above foods were stored in this unit for less than 2 hours. Potentially hazardous foods (time/temperature conrol for safety food) shall be held cold at a temperature of 41°F or below.

    Observation: There is no Certified Food Manager present at the beginning of the inspection.
    Correction: The Certified Food Manager arrived approximately 10 minutes after the beginning of the inspection. It shall be unlawful to operate a food establishment unless it is under the immediate control of a certified food manager. It is highly suggested that you have more than one employee with the food manager's license. ORS Interactive, Inc. (see handout provided) issues the required photo identification card with proof of successful completion of a certified food manager's exam. Failure to have a certified food manager on site during ALL hours of operation including food preparation, food service, and cleaning/sanitizing of equipment/utensils will result in closure of the establishment.

    Observation: The person in charge (PIC) has a valid certificate of successful completion of a Certified Food Manager (CFM) exam but does not possess the CFM card issued by ORS Interactive, Inc.
    Correction: Food managers who possess a valid certificate of passing a food safety exam shall obtain the photo identification card from ORS Interactive, Inc. (see handout provided). A copy of the photo identification card can be faxed to the food safety section at (703) 653-9448. Failure to provide a copy of the photo identification card may result in further enforcement action.

    Observation: Observed that hand soap was not provided at the back handwashing sink.
    Correction: A foodworker placed handsoap into a soap dispenser and placed at the handsink. Provide hand soap at each hand sink to allow employees to properly wash their hands.

The purpose of this inspection is to conduct a routine inspection. Thank you for following me through the inspection as it allows for questions to be asked and clarification of any issues that arise. Employee health policy in place. Chemical sanitizing dishmachine measured within an acceptable limit. Quaternary ammonium sanitizer dispensed at the three vat sink measured at an acceptable limit. Observed several employees wash hands during today's visit.

1. Within 10 days, repair the Superior two door prep cooler so that it is capable of maintaining potentially hazardous foods at 41f or below. Fax or email a copy of the invoice for the repair/adjustment to my attention at the health department. Please ensure that the name and address of this facility is on the fax or email.     Observation: A food employee was observed handling the following ready-to-eat food using their bare hands: swiss cheese on a sandwich.
    Correction: Discard the cheese. Except when washing fruits and vegetables, food employees may not contact ready-to-eat foods using their bare hands. Employees shall use suitable utensils such as deli tissue, spatulas, tongs, or clean disposable gloves to handle all ready-to-eat foods.

    Observation: The following food item(s) were observed cold holding at improper temperatures using a calibrated food temperature measuring device: saute mushrooms 52f, provolone cheese 52f, raw hamburger 53f inside the Superior two door prep cooler 59F.
    Correction: Because these foods were held inside this unit for less than 4 hours, they were relocated to the walk in refrigerator to cool and hold until the repair of the prep cooler is completed. Potentially hazardous foods (time/temperature control for safety food) shall be held cold at a temperature of 41°F or below unless the permit holder is using "time as public health control" as specified under 3-501.19 to limit bacteria growth.

    Observation: The following refrigeration unit is not operating properly and is unable to maintain cold food at or below 41°F: Superior two door prep cooler 59F.
    Correction: Within 10 days, repair/adjust the unit so it is able to maintain foods at or below 41°F. Do not use the unit to store potentially hazardous foods while it is not operating properly. Manager called for service.

The purpose of this visit was to conduct a risk factor inspection. Environmental health specialist (EHS) discussed replacing gaskets on refrigeration units and food on floor inside of walk-in cooler. EHS provided additional training materials in English and Spanish for proper cooking temperatures for new employees.
  • Critical: Handwashing / When to wash hands (corrected on site)
    Observation: A food employee failed to wash his or her hands before engaging in food preparation, before putting on gloves, or after engaging in any activity that could contaminate the hands.
    Correction: ALL food employees shall wash their hands OFTEN especially after handling trash, using the restroom, handling raw meats, handling soiled equipment and utensils, BEFORE putting on clean gloves, etc. Proper handwashing frequencies will aid in the reduction of foodborne illness.

    Observation: Observed that the floor tiles in kitchen are missing, broken and not maintained in good repair. Light bulbs observed blown out near slicer and cold handle on handsink located by three vat sink is missing.
    Correction: Repair or replace the floor tiles, light bulbs and cold water handle. Repair and replace to allow for the functionality of the physical facilities.

The purpose of today's visit was to conduct a risk factor assessment inspection. EHS provided manager with and discussed the following handouts:-----sample menu showing a complete consumer advisory, CFM guidelines, Food for Thought Newsletter titled "How Cold Is It", Equipment Monitoring Log, Food Temperature Log, food safety and glove usage and hand washing sign.
Please fax or email the following within ten calendar days:
1) A third party service report for the Cookline cooler (Superior 2dr prep),
2) One of the CFM's photo card.
3) The updated daily menu showing that burgers have a complete consumer advisory (since per Manager burgers is always on daily menu)
Please add the asterisk to the grilled salmon on the main menu within 30 calendar days and email a photograph of the updated menu.

The single-use gloves shall be discarded when damaged or soiled or when interruptions occur in the operation. DISCUSSED WITH MANAGER AND FOOD EMPLOYEE. FOOD EMPLOYEE REMOVED GLOVES, WASHED HANDS AND PUT ON A NEW PAIR OF GLOVES.

    Observation: A review of the daily special menu with the foodservice operator indicates that there is no consumer advisory for the following food item(s) that may be served raw and/or undercooked:-----------salmon and burgers. Also on the main menu, there is no disclosure asterisk for the grilled salmon sensation.
    Correction: If an animal food such as beef, eggs, fish, lamb, milk, pork, poultry, or shellfish is served or sold raw or undercooked the permit holder shall inform consumers of the significantly increased risk of consuming such foods by way of a disclosure and reminder using brochures, deli case or menu advisories, label statements, table tents, placards, or other effective written means.

The purpose of this visit was to conduct a risk factor inspection.
EHS provided cross contamination, cooling and cooking information in English and Spanish. EHS provided glove use information.
Employee health information was provided in English and Spanish. Manager will conduct training and provide documentation of training to the health department by June 28, 2013.
EHS discussed apron use, air drying, replacing gaskets on refrigeration units, replacing cutting boards, replacing floor tiles in the kitchen, cleaning floors, walls and ceilings, stored of food in walk-in freezer and walk-in cooler.

A complete employee health policy is required to be in place at the food establishment. At the time of this inspection, the Health Department provided and reviewed handouts and resource information in a red folder labeled "Employee Health Policy.
    Correction: A complete employee health policy must have the following elements:
    1) Employee training on foodborne illness, particularly symptoms of illness and prevention of the Big Five illnesses (see "What is Foodborne Illness" handout)

    Observation: There is no Certified Food Manager present at the beginning of the inspection.
    Correction: It shall be unlawful to operate a food establishment unless it is under the immediate control of a certified food manager. It is highly suggested that you have more than one employee with the food manager's license. ORS Interactive, Inc. (see handout provided) issues the required photo identification card with proof of successful completion of a certified food manager's exam. Failure to have a certified food manager on site during ALL hours of operation including food preparation, food service, and cleaning/sanitizing of equipment/utensils will result in closure of the establishment.

  • Critical: Responsibility of PIC to Require Food Employees to Report GI Symptoms or Cuts, Disease, Medical Condition or Exposure to PIC (corrected on site)
    Observation: Upon discussion with the person-in-charge, one or more of the elements of an effective employee health policy is either missing or incomplete. A complete employee health policy is required to be in place at the food establishment. At the time of this inspection, the Health Department provided and reviewed handouts and resource information to be used by the person-in-charge to develop a complete employee health policy.
    Correction: A complete employee health policy must have the following elements:
    1) Employee training on foodborne illness, particularly symptoms of illness and prevention of the Big Five illnesses (see "The Big Five Foodborne Illnesses Information Sheet" handout)
  • Critical: Eating, Drinking, or Tobacco Use / Proper Food Employee Drinking Containers (corrected on site)
    Observation: A food handler was observed drinking from an uncovered beverage container while in the kitchen.
    Correction: Food employees may drink only from a covered container using a straw.
  • Critical: Package Integrity & No Dented Cans/ Specifications for Receiving (corrected on site)
    Observation: The following food items from damaged packaging were found offered for sale or service: Observed rusted, expired cans of chunky applesauce, kidney beans and pitted olives in dry storage area. Discarded.
    Correction: Food packages such as canned goods shall be in good condition and protect the integrity of the contents so that the food is not exposed to adulteration or potential contaminants. Return all dented and damaged canned goods to your food supplier for a credit.
  • Critical: Food Protection from Contamination / Separate Raw from RTE (corrected on site)
    Observation: Raw animal food holding in a manner that may cause cross contamination of ready-to-eat food as follows: Observed raw ground sausage stored over soups and raw beef stored over containers of dressings.
    Correction: Food shall be protected from cross contamination by separating raw animal foods like poultry, beef, pork, seafood, and eggs during holding from ALL other foods such as cooked foods, ready-to-eat foods, washed produce, etc.
  • Food Storage / Preventing Contamination
    Observation: Food stored on the floor and/or food stored less than 6 inches off the floor. walk-in refrigerator and dry storage area.
    Correction: Food shall be protected from contamination by storing the food at least 6 inches off the floor on approved shelving units or dunnage racks. Milk crates, soda crates, or bread racks are not suitable for food storage.
  • Cooling Methods (corrected on site)
    Observation: The following methods used for cooling were not adequate to facilitate proper cooling: Observed mashed potatoes in a plastic bag at 92F.
    Correction: Cooling hot foods shall be accomplished as follows: 1) placing the food in shallow pans
  • Critical: Cold Holding of Potentially Hazardous Food at 41°F or less (corrected on site) (repeated violation)
    Observation: The following food items were observed cold holding at improper temperatures using a calibrated food temperature measuring device: Cooked onion 44F, raw steak 44F -- Superior 2 door refrigerator. Per chef the items were used and were left out for some time.
    Correction: Potentially hazardous foods (time/temperature control for safety food) shall be held cold at a temperature of 41°F or below unless the permit holder is using "time as public health control" as specified under 3-501.19 to limit bacteria growth.
  • Critical: Date Marking of Refrigerated, Ready-to-Eat PHF (TCS Food) (corrected on site)
    Observation: The following refrigerated, ready-to-eat, potentially hazardous food that is commercially processed is not used or discarded within 24 hours of when the original container was opened and is not observed to be date marked: Provolone cheese and american cheese -- superior 2 door prep refrigerator.
    Correction: Refrigerated, ready-to-eat, potentially hazardous commercially processed foods when opened and kept for more than 24 hours cannot have a shelf-life of more than 7 days, including the day of the original container was opened. These food items must be marked with either the "consume-by" date and/or "preparation" date, color-coded labels, or another marking system/method that effectively indicates shelf-life that is clearly understood and properly used by the employees. This date cannot exceed any manufacturer determined "use-by" date based on food safety.
  • Equipment / Non Food / Design / Easy Clean
    Observation: Soda crates found used for the following purposes: Eleavate food in freezer.
    Correction: Milk crates, soda crates, and/or bread racks are not designed to store or elevate food, equipment, utensils, clean linens, and single-service items. Provide only ANSI-certified (such as NSF-listed) shelving units, storage units, or dunnage racks.
  • Temp Meas Devices for Cold & Hot Hold Equipment, Present
    Observation: There was no temperature measuring device located in the following cold or hot holding equipment: walk-in, superior 2 door refg, True 2 door prep refrigerator, True 1 door display refrigerator.
    Correction: Cold or hot holding equipment used for the storage of potentially hazardous foods (time/temperature control for safety food) shall be designed to include and shall be equipped with at least one integral or permanently affixed temperature measuring device that is located to allow easy viewing of the device's temperature display.
  • Equipment / Good Repair / Components / Gaskets
    Observation: The door gaskets of the following unit is damaged: Walk-in refrigerator.
    Correction: Equipment components such as doors, seals, hinges, fasteners, and kick plates shall be kept intact, tight, and adjusted in accordance with manufacturer's specifications.
  • Critical: Equipment / Food-Contact / Visibly Clean (corrected on site)
    Observation: The following equipment/utensils were observed soiled to sight and touch: Meat slicer.
    Correction: Equipment food-contact surfaces and utensils shall be clean to the sight and touch. A surface that is soiled cannot be effectively sanitized.
  • Handwashing Signage at Handwashing Facilities Required (corrected on site)
    Observation: Observed that sign or poster that notifies food employees to wash their hands is not provided at all handwashing sinks used by food employees.
    Correction: Provide a sign or poster at all handwashing sinks used by food employees that reminds food employees to wash their hands.
  • Physical Facilities Good Repair
    Observation: Observed that the tiles in the kitchen is missing and broken, handsink next to 3-vat sink is broken, holes in wall in dry storage area, hole in wall in women restroom, caulking needed in the women restroom handsink.
    Correction: Repair the physical facilities. Poor repair and maintenance compromises the functionality of the physical facilities.
  • Controlling Pests by Eliminating Harborage Conditions
    Observation: Harborage conditions exist in dry storage area and kitchen. Also noted lot of mice droppings in the dry storage area.
    Correction: Insects and other pests are capable of transmitting disease to people by contaminating food and food-contact surfaces. Eliminate harborage conditions to control and prevent the presence of pests on the premises.
  • Cleaning Frequency for Physical Facilities
    Observation: Observed that the gaskets of refrigerators, dry storage area (mice dropping all over even on the bottom shelves), under the sinks in bar area is in need of cleaning.
    Correction: Clean the physical facilities at a frequency necessary to ensure the protection and safe preparation of food.
  • Mops in Air-dry Position
    Observation: Observed that mops are improperly stored between use.
    Correction: Store mops in a position that allows them to air dry without soiling walls, equipment, or supplies.
