The purpose of this visit was to perform a routine inspection.
- Food Temperature Measuring Device Required & Readily Accessible
Observation: The certified food manager could not provide a food temperature measuring device.
Correction: A metal-stemmed probe food thermometer shall be provided and readily accessible for use in ensuring attainment and maintenance of food temperatures.
- Critical: Quaternary Ammonium Compound Sanitizer Solution Concentration per Use Directions (corrected on site)
Observation: When tested, the quaternary ammonium compound-based sanitizing solution was observed to be at a concentration less than what is required for effective sanitization of food-contact surfaces of equipment and utensils. Observed the sanitizing solution in the 3-vat sink with a concentration of 0ppm total quaternary ammonium compound.
Correction: The quaternary ammonium sanitizer used in this facility shall have a minimum concentration of at least 200 ppm based on the manufacturer's use directions and using the appropriate test kit.MANUALLY PREPARED SANITIZING SOLUTION
- Certified Food Manager/Certificate Process
Observation: The person in charge (PIC) has a valid certificate of successful completion of a Certified Food Manager (CFM) exam but does not possess the CFM card issued by ORS Interactive, Inc.
Correction: Food managers who possess a valid certificate of passing a food safety exam shall obtain the photo identification card from ORS Interactive, Inc. (see handout provided). A copy of the photo identification card can be faxed to the food safety section at (703) 653-9448. Failure to provide a copy of the photo identification card may result in further enforcement action.

- Cleaning Frequency for Physical Facilities
Observation: Observed that the cover over the exhaust at the ceiling near the door has visible dirt and dust and is in need of cleaning.
Correction: Clean the physical facilities at a frequency necessary to ensure the protection and safe preparation of food. ASKED THE CFM AND CENTER DIRECTOR TO CLEAN THE EXHAUST GRILL COVER AT REGULAR INTERVALS.

Observation: The concentration of the Quarternary Ammonium sanitizer solution was measured at >500 ppm when tested with a quat test strip. This concentration exceeds the approved limits for use with food-contact surfaces of equipment and utensils in a foodservice establishment.
Correction: Maintain the concentration of Quaternary solution at 200-400 ppm. Verify concentration using the appropriate test kit. CORRECTED: STAFF ADJUSTED THE CONCENTRATION OF THE SOLUTION IN THE 3 VAT SINK TO MEASURE AT BETWEEN 300-400 PPM WHEN TESTED WITH THE TEST STRIP

Observation: Employees observed working in the food service area without proper hair restraints.
Correction: Food employees shall wear hair restraints such as hats, visors, hair nets or beard nets to effectively keep their hair from contacting exposed food, clean equipment, utensils, linens, and unwrapped single-service items. This section does not apply to food employees such as counter staff who ONLY serve beverages and wrapped or packaged foods, hostesses, and wait staff.

Observation: There is no covered refuse container for the disposal of feminine napkins in the ladies room stall.
Correction: A toilet room used by females shall be provided with a covered receptacle for sanitary napkins.
- Physical Facilities Good Repair
Observation: Observed that the tray table holding one of the Vulcan ovens is rusted in several areas and not maintained in good repair.
Correction: Repair the physical facilities and equipments. Poor repair and maintenance compromises the functionality of the physical facilities.

Observation: Observed that the insides and the outer surfaces of the vulcan ovens are in need of cleaning.
Correction: Clean the physical facilities at a frequency necessary to ensure the protection and safe preparation of food.
