Today I conducted a routine inspection and discussed with the certified food manager the following: 1. Management should reiterate the 5 major risk factors associated with foodborne illness with all staff frequently: Employee health policy, Personal hygiene, Contaminated equipment/utensils, Time/Temperature relationships for food and Unapproved food sources. 2. Management should calibrate the metal stemmed food thermometer at least once a week in an ice water bath to 32f to ensure accuracy. Wash, rinse and sanitize the thermometer before each use. 3. Management should renew any expired LCFM cards at ORS Interactive. See hand out provided. 4. Management should remind staff to keep any hoses above the flood level rim of the mop sink basin. - Food on Display Protected From Contamination
Observation: The food on display is not protected from contamination. Observed samples of smoothies unprotected.
Correction: Food on display shall be protected from contamination by the use of packaging, counter, service line, or salad bar food guards, display cases, or other effective means. DISCUSSED WITH MANAGER. MANAGER WILL ORDER A COVER TO PROTECT SAMPLES FROM POSSIBLE CONTAMINATION.
- Temp Meas Devices for Cold & Hot Hold Equipment, Present
Observation: There was no temperature measuring device located in the following cold or hot holding equipment: 1. Randell 2DR flat top refrigerator (wheat grass), 2. Master Bilt freezer, 3. Beverage Air 2DR flat top refrigerator (produce), 4. Beverage Air 2DR flat top refrigerator (soy and almond milk).
Correction: Cold or hot holding equipment used for the storage of potentially hazardous foods (time/temperature control for safety food) shall be designed to include and shall be equipped with at least one integral or permanently affixed temperature measuring device that is located to allow easy viewing of the device's temperature display. DISCUSSED WITH MANAGER. MANAGER SHALL ORDER AND INSTALL NEW AMBIENT AIR THERMOMETERS INSIDE ALL REFRIGERATION AND FREEZER UNITS. INSTALL THE THERMOMETERS IN THE WARMEST LOCATION OF THE UNITS (near the doors).

Observation: Light bulb(s) inside the True 1DR refrigerator for self service beverages is not covered by a protective shielding.
Correction: Provide light bulbs that are shielded, coated, or otherwise shatter-resistant in areas where there is exposed food, clean equipment, utensils, linens, or unwrapped single-service and single-use articles. DISCUSSED WITH MANAGER. MANAGER WILL PROVIDE A PROTECTIVE SLEEVE AROUND THE LIGHT BULB TO PREVENT POSSIBLE PHYSICAL CONTAMINATION.
- Controlling Pests by Eliminating Harborage Conditions
Observation: Observed fruit flies in the facility.
Correction: Insects and other pests are capable of transmitting disease to people by contaminating food and food-contact surfaces. Eliminate harborage conditions to control and prevent the presence of pests on the premises. DISCUSSED WITH MANAGER AND HE IS WORKING WITH A LICENSED PEST CONTROL COMPANY TO ELIMINATE THE ISSUE.

Observation: The certified food manager could not provide a small diameter food temperature measuring device.
Correction: A small diameter metal-stemmed probe food thermometer shall be provided and readily accessible to accurately measure the temperature of thin foods such as meat patties and fish filets.
- Equipment / Non-food Contact / Visibly Clean
Observation: The nonfood-contact surfaces of the following equipment were observed soiled with an accumulation of dust, dirt, food residue, and/or other debris: gaskets on all refrigeration units
Correction: Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue and other debris to maintain a sanitary work environment and prevent pest attraction.
- Clean Ice Bins, Beverage Dispenser Nozzles, Etc. per Manufacturer Specs.
Observation: Surfaces of ice bins and enclosed components of equipment are not being cleaned as required.
Correction: Surfaces of utensils and equipment contacting food that is NOT potentially hazardous (time/temperature control for safety food) such as ice bins, beverage dispensing nozzles, and enclosed components of equipment such as ice makers, cooking oil storage tanks and distribution lines, beverage and syrup dispensing lines or tubes, coffee bean grinders, and water vending equipment at a frequency specified by the manufacturer or at a frequency necessary to preclude accumulation of soil or mold.
- Critical: Certified Food Manager/Presence Required (corrected on site)
Observation: There is no Certified Food Manager present at the beginning of the inspection.
Correction: It shall be unlawful to operate a food establishment unless it is under the immediate control of a certified food manager. It is highly suggested that you have more than one employee with the food manager's license. ORS Interactive, Inc. (see handout provided) issues the required photo identification card with proof of successful completion of a certified food manager's exam. Failure to have a certified food manager on site during ALL hours of operation including food preparation, food service, and cleaning/sanitizing of equipment/utensils will result in closure of the establishment.
- Physical Facilities Good Repair
Observation: Observed that the masking tape observed holding doors of freezer together and broken thermometer inside of Bev air 2dr unit #1 are not smooth and easily cleanable and are not maintained in good repair.
Correction: Repair the physical facilities. Poor repair and maintenance compromises the functionality of the physical facilities.

Observation: Methods are not being used to control gnats, fruit flies and pests throughout the facility.
Correction: Insects and other pests are capable of transmitting disease to people by contaminating food and food-contact surfaces. If pests are found on the premises methods shall be used to control their presence.
- Critical: Common Name on Working Containers of Toxics (Cleaners, Sanitizers) (corrected on site)
Observation: Working containers of poisonous and toxic materials are not properly labeled with a common name.
Correction: Label all working containers of poisonous and toxic materials, including cleaners and sanitizers, with a common name to prevent the dangerous misuse of these materials.
