The above observations and the following were discussed with the person in charge:
-Replace the lids/containers covering the foods on the serving line that do not completely close.
-Seal around the pipe under the handsink in the back of the kitchen.

  • Temperature Measuring Devices (corrected on site)
    Observation: No temperature measuring device located in the under counter reach in coolers. The PIC stated the thermometers are on order.
    Correction: Ensure all coolers are equipped with temperature measuring devices.
  • Equipment - Good Repair and Proper Adjustment
    Observation: Observed a large accumulation of ice build up in the walk in cooler which could prevent the equipment from working properly.
    Correction: Ensure the ice build up is removed from the pipes and compressor in the walk in cooler to allow the equipment to function properly.
  • Equipment and Utensils, Air-Drying Required (repeated violation)
    Observation: Observed stainless steel food storage containers stacked while wet after cleaning and chemical sanitization.
    Correction: Ensure all equipment is either air dried or dried with a paper towel prior to stacking after cleaning and chemical sanitization.

The above observations and the following were discussed with the person in charge:
-Grout between the floor tiles in front of the fryer and rethermalizer is no longer present. Grout must be provided in this area as soon as possible.
-Additional backflow prevention device required on the splitter on the mop sink faucet. Handout provided.

-All in use utensils are washed, rinsed and sanitized at least once every 4 hours.
Excellent handwashing procedures observed by all employees throughout the inspection, great job!

    Observation: Single service items observed unprotected from contamination. Observed single service cups stored by the handsink. The handsink has no splash guard and cups were observed to have water on them from the handsink. The PIC discarded the plastic cups during the inspection and will have them placed in another area until a splash guard can be installed.
    Correction: Store single service items in its original protective packaging or inverted in an approved dispenser.
  • Physical Facilities in Good Repair
    Observation: The following areas are not maintained in good repair: 1. a hole in the wall was observed by the register 2. gaps observed around the plumbing in the wall under the handsink.
    Correction: Maintain physical facilities in good repair. Poor repair and maintenance compromises the functionality of the physical facilities.

The above observations and the following were discussed with the person in charge:
-Water observed in the bottom of the under counter reach in cooler by the drive thru. Please remove the water and monitor this cooler to ensure temperatures are maintaining 40'F and below.
-Clean the upsplash of the Fruitista Machine.
-Clean the floors under the shelves at the front counter.
-Quaternary ammonium sanitizer in the 3 compartment sink and in the sanitizer buckets measured at 200 ppm.
-Employee health policy observed.
-Please replace the thermometers in the under counter reach in coolers on the line.
Facility observed to be very clean and well maintained, excellent job by all staff members!

No violation noted during this evaluation.

The above observations and the following were discussed with the person in charge:
-Employee health policy observed.
-Work orders are in place for the ice build up on the lines in the walk in freezer and for the leak in the lobby carbonated beverage dispenser.
-All in use utensils are being washed, rinsed and sanitized or changed out at least once every 4 hours.
-Quaternary ammonium sanitizer in the 3 comaprtment sink measured at 200 ppm.
The person in charge was very knowledgeable regarding safe food handling practices and willing to make several corrections throughout the inspection, thanks!

    Observation: Surfaces of the soda nozzles on the carbonated beverage dispenser in the lobby and in the drive thru were observed soiled with accumulations of grime and debris. PIC had all soda nozzles cleaned and sanitized during the inspection.
    Correction: Clean the surfaces of the soda nozzles at any time when contamination may have occurred, at least every 24 hours.
  • Equipment and Utensils, Air-Drying Required (corrected on site) (repeated violation)
    Observation: Stainless steel containers were found stacked while wet after cleaning and chemical sanitization. PIC had an employee dry the containers with a paper towel during the inspection.
    Correction: Items must be allowed to drain and air-dry before being stacked or stored to allow evaporation of chemical sanitizer and moisture that may encourage microbial growth.
  • Refuse - Maintaining Refuse Areas and Enclosures
    Observation: Grease was observed on the ground adjacent to the refuse container and grease barrel outside of the facility.
    Correction: The refuse container storage area and grounds adjacent to the containers are to be maintained clean and sanitary.

The above observations and the following were discussed with the person in charge:
-Employee health policy observed and discussed.
-Please cover the light bulb in the outside storage area.
-Facility will be replacing the 3 compartment sink soon. The existing plumbing connection under the 3 compartment sink does not provide an air gap/break. Please contact the health department prior to installing the new sink for approval.
-Quaternary ammonium sanitizer in the 3 compartment sink and in the sanitizer buckets was measured at 200 ppm.
Lots of good handwashing and glove use observed during today's inspection, great job by all staff members!

    Observation: Surfaces of the soda nozzles were in contact with non-potentially hazardous food items that were observed soiled with accumulations of grime and debris. PIC stated the soda nozzles are being soaked in sanitizer each night. Advised PIC to also scrub the soda nozzles to prevent the excess build up. PIC removed the soda nozzles and had them cleaned during the inspection.
    Correction: Clean the surface of the soda nozzles at any time when contamination may have occurred, at least every 24 hours, before restocking the consumer self-service equipment/utensils, or per manufacturer specifications.
  • Equipment and Utensils, Air-Drying Required
    Observation: The stainless steel pans stored on the storage rack were found stacked while wet after cleaning and chemical sanitization.
    Correction: Items must be allowed to drain and air-dry before being stacked or stored to allow evaporation of chemical sanitizer and moisture that may encourage microbial growth.
  • Plumbing System Maintained in Good Repair
    Observation: The plumbing in several areas throughout the kitchen is in poor repair. Observed the faucets leaking at the 3 compartment sink, prep sink and at the mop sink. Observed a leak in the pipes in the walk in freezer causing a significant accumulation of ice build up in this area. Observed standing water around the carbonated beverage dispenser in the lobby and around the carbonator in the storage area in the back of the kitchen.
    Correction: Repair and maintain all plumbing components and fixtures.
  • Plumbing System Maintained in Good Repair
    Observation: Plumbing connections under the 3 compartment sink are leaking.
    Correction: Plumbing systems and components shall be maintained in good repair.
  • Outer Openings - Protected (repeated violation)
    Observation: Outer opening of the food establishment is not protected against entry of insects and rodents. Observed a gap along the side of the back door of the establishment.
    Correction: Protect the food establishment against the entry of insects and rodents by 1. Filling or closing holes and other gaps along floors, walls, and ceilings, 2. Closed, tight fitting windows, and 3. Solid, self-closing, tight-fitting doors. Insects and rodents are vectors of disease-causing microorganisms which may be transmitted to humans by contamination of food and food-contact surfaces.
  • Physical Facilities - Cleaning Frequency and Restrictions
    Observation: The following areas are noted in need of cleaning: 1. the floors around and behind the hot water heater 2. the floors throughout the dry storage area including under the shelves 3. the floors around the fryer 4. the floors under and behind the carbonated beverage dispenser in the lobby (due to leak in carbonated beverage dispenser).
    Correction: All floors, walls, and ceilings must be cleaned as often as necessary to keep them clean. Cleaning of the physical facilities is an important measure in ensuring the protection and sanitary preparation of food. A regular cleaning schedule should be established and followed to maintain the facility in a clean and sanitary manner.
