- Plumbing System Maintained in Good Repair
Observation: Plumbing connections under the cookline and the bar hand sinks sink are leaking.
Correction: Plumbing systems and components shall be maintained in good repair.
- Light Bulbs Protective Shielding
Observation: Light bulb in a ceiling mounted light fixture on the cookline was noted not shielded, coated, or otherwise shatter-resistant.
Correction: Shield or replace light bulb with a coated or shatter-resistant bulb.
- Outer Openings - Protected
Observation: Outer opening of the food establishment is not protected against entry of insects and rodents ( rear side exit door was observed propped open, during today's inspection ( the side exit leads into an open air trash and general storage area).
Correction: Protect the food establishment against the entry of insects and rodents by 1. Filling or closing holes and other gaps along floors, walls, and ceilings, 2. Closed, tight fitting windows, and 3. Solid, self-closing, tight-fitting doors. Insects and rodents are vectors of disease-causing microorganisms which may be transmitted to humans by contamination of food and food-contact surfaces.
- Critical: Toxics - Common Name/working Containers of Toxics*
Observation: A working container of grill cleaner was not properly labeled ( a citrus concentrate (lemon/lime juice) was observed re-used to store grill cleaner, in the rear chemical storage area reused ).
Correction: Working containers of toxic items are to be properly labeled with contents. Identification of these containers with the common name of the material helps prevent the dangerous misuse of the contents.
- Toxics - Separation of Toxics (corrected on site)
Observation: Containers of cleaners are not properly stored to prevent the contamination of food, equipment, utensils, linens or single service items ( a bottle of grill cleaner was observed stored with cider vinegar on a bottom shelf in the side prep/canned storage area).
Correction: Containers of cleaners must be separated by spacing or partitioning to prevent contamination of food, equipment, utensils, linens or single service items.

Observation: Employees or applicants are not aware of the reporting procedures concerning information about their health and activities as they relate to diseases that are transmissible through food, including the date of onset of jaundice or of an illness due to Salmonella Typhi, Shigella spp., Escherichia coli O157:H7, Hepatitis A virus or Norovirus
Correction: Have the license holder require employees or applicants report to the person in charge any information about their health and activities if they are diagnosed with or had a past illness from Salmonella Typhi, Shigella spp., Escherichia coli O157:H7, Hepatitis A virus or Norovirus

Observation: In use open drinking containers stored in a manner that may contaminate food, food contact surfaces or utensils ( open employee drinks were observed on the cookline and the side prep area).
Correction: Employees may drink from a closed beverage container if the container is handled to prevent contamination of (1) The employee's hands (2) The container and (3) Exposed food clean equipment, utensils, and linens and unwrapped single-service and single-use articles.
- Critical: Food - Potentially Hazardous Food - Cold Holding*
Observation: Portioned salsa in the wait Silver King undercounter cooler, cheese sauce (in large plastic containers) and raw beef/chicken/steak and raw scallops in the grill refrigerated drawers were cold holding at improper temperatures (portioned salsa-44F, raw beef/raw chicken-51F, raw steak-54F & raw scallops-53F). The operator adjusted the thermostat on the grill refrigerated drawer unit.
Correction: Cold hold potentially hazardous food at 41°F or below to inhibit the growth of harmful bacteria.
- Critical: Food-PHF/TCS-Cold Holding-shell eggs
Observation: Observed raw shell eggs not stored in refrigeration equipment that maintains ambient air temperature of 45°F or less ( a flat of raw shelled eggs were observed stored on shelf at ambient air at the RT side line station)
Correction: Store raw shell eggs in refrigerated equipment capable of maintaining ambient air temperatures of 45°F or less
- Temperature Measuring Devices
Observation: There was no temperature measuring device located in the grill refrigerated drawer unit and line maketable unit.
Correction: Provide a temperature measuring device in all hot or cold holding equipment used to store potentially hazardous foods so that employees can routinely monitor the ambient air temperature.

Observation: The following equipment food-contact surfaces were observed soiled to sight and touch: the side prep slicer and standing mixer.
Correction: Clean and sanitize these surfaces for food contact.
- Equipment and Utensils, Air-Drying Required
Observation: Pans were found stacked while wet after cleaning and chemical sanitization, on the side drying shelves..
Correction: Items must be allowed to drain and air-dry before being stacked or stored to allow evaporation of chemical sanitizer and moisture that may encourage microbial growth.
- Refuse - Maintaining Refuse Areas and Enclosures
Observation: Trash and litter were observed adjacent to the refuse container outside the facility. Also noted the outside dumpster was overflowing with trash and debris ( the dumpster area is adjacent to he rear exit/employee entrance door)
Correction: The refuse container storage area and grounds adjacent to the container are to be maintained clean and sanitary.
- Critical: Pests-Controlling Pests*
Observation: Observed methods are not being used to control pests (live flies were observed on the cookline, wait station area, and side prep area). The rear exit/employee entrance door was observed ajar with the exit air curtain not operable earlier)
Correction: The presence of insects, rodents, and other pests shall be controlled to minimize their presence on the premises by using methods such as trapping devices or other methods as required.
