• 0130 - Corrected During Inspection Critical Food employee observed with soiled hands and arms. Observed employee handle raw meat and not wash his hands before doing other tasks in the food prep area.
  Inform all food employees to keep their hands and exposed portions of their arms clean. Discussed with PIC.
• 0960 2 - The food contact surface of the broken trays and lids are not durable, easily cleanable, and/or resistant to pitting.
  Replace the trays and lids to permit easy cleaning and prevent the migration of deleterious substances, or transference of colors, odors, or tastes to food.
• 3020 - Corrected During Inspection Soap was not provided at the hand washing lavatory next to the 3 compartment sink. Dispenser broken.
  Hand soap must be provided at all hand washing lavatories to encourage proper hand washing. Hand cleanser must always be present to aid in reducing microorganisms and particulate matter found on hands.
• 3030 - Corrected During Inspection No disposable towels were provided at the hand washing lavatory next to the 3 compartment sink.
  Hand drying devices such as individual disposable paper towels, a continuous towel system that supplies the user with a clean towel or heated air hand drying device must be provided at all hand washing lavatories to encourage proper hand washing and avoid employees to drying their hands on their clothing or other unclean materials

March 09, 2006 (Routine)

• 0770 - Frozen foods not maintained frozen. Reach-In freezer observed operating at 35 degrees F inside. Outside gauge reading 1 degrees F.
  Ensure frozen food is maintained frozen. Have unit serviced immediately. Pull product and do not use until repaired.
• 1320 - Repeat There was no temperature measuring device located in both prep units. Also observed broken thermometer in small reach-in next to drive-thru window.
  Provide a temperature measuring device in all hot or cold holding equipment used to store potentially hazardous foods so that employees can routinely monitor the ambient air temperature.
• 1450 - Repeat Prep refrigerators are not sufficient in capacity to meet the food storage demands of the establishment. Observed both units operating at an elevated level. Food temperatures on top were below 45 degrees F.
  Have both units checked while repairman is fixing freezer. Improper food storage temperatures are a major contributing factor to foodborne illness. Inside of units should be 41 degrees F or below.
• 1780 - Critical Ice machine had mold growth.
  Clean the surface of ice machine at any time when contamination may have occurred, or per manufacturer specifications to prevent mold growth.

June 18, 2004 (Routine)

• 0820 - Critical Repeat Condiments and other foods in top trays of both sandwich prep coolers cold holding at improper temperatures.
  Relocate food to a refrigeration unit that is capable of maintaining food storage at 41F or below, or adjust thermostat settings of both coolers to assure holding temperatures of 41F or below.
• 2930 - Outer opening (drive thru window) of the food establishment is not protected against entry of insects.
  Protect the food establishment against the entry of insects by installing an automatic self-closing window or air curtain. Flies are common in the area during the spring and summer months. Insects and rodents are vectors of disease-causing microorganisms which may be transmitted to humans by contamination of food and food-contact surfaces.
• 2720 - Repeat Dumpster or outside refuse container was open.
  Cover all waste containers when not in continuous use.
• 1320 - There was no operable and adequate temperature measuring device located in the dining room meat cooler.
  Provide a temperature measuring device in all hot or cold holding equipment used to store potentially hazardous foods so that employees can routinely monitor the ambient air temperature.
• 3660 - Establishment operating without a valid permit.
  Cease all food service operations until you receive a permit to operate from the health department. Expired permit from 2003 has not been renewed, in spite of numerous notifications.
