Observation: No single-use toweling or other hand drying device available at handwashing sink for hand drying. The wipes that you are currently using in place of single use toweling are not acceptable.
Correction: Provide single-use toweling or other approved devices at handwashing sink to facilitate proper handwashing.
- IN-USE UTENSILS, BETWEEN USE STORAGE (corrected on site)
Observation: Ice scoop was improperly stored laying on a towel on top of the ice bin.
Correction: Store in-use utensils in the food item with handle extended, on a clean surface, running dipper well or in a container of water greater than 135°F. Ice scoop was placed in the ice bin with the handle extended.
- EQUIPMENT AND UTENSILS - NONFOOD CONTACT SURFACES - CONSTRUCTED
Observation: The wood shelf above the 3 compartment sink is not easily cleanable. Remove or coat the wood to make it smooth and nonabsorbent.
Correction: Provide non-food contact surfaces that are constructed of a corrosion resistant, nonabsorbent, and smooth materials.
- CATEGORY ASSIGNMENT
Observation: Food establishment is operating beyond their current food permit category assignment. The facility has been serving hot dogs on the grill outdoors for special events.
Correction: Reduce current operation to meet current license restrictions or modify your current license and pay applicable fees to the department. All food code requirements for the new risk category shall be met before moving from one category to another.

Observation: Person in charge has not informed or required food employees to report signs, symptoms, or diagnosis of foodborne illness.
Correction: Complete employee health forms for all employees and keep on file. Inform food employees of the requirements to report signs, symptoms and diagnosis of foodborness illness to the person in charge.

Observation: No air gap provided on the ice bin drain line and soda gun drain line.
Correction: Provide an air gap on water supply side to protect water supply.

Observation: Plumbing system not provided with an air gap or backflow prevention at the hose in the basement crawl space area.
Correction: Provide air gap or backflow prevention device. Plumbling systems shall be provided with backflow prevention at each point of use in the food establishment.

- EQUIPMENT AND UTENSILS CONFORM TO ANSI STANDARDS
Observation: Konop meat sticks stored in residential refrigerator. All equipment used to store potentially hazardous foods must be commercial grade and approved for that use. Store the meat sticks in a commercial refrigerator.
Correction: Unapproved equipment shall be removed from food service.
- SINGLE-USE AND SINGLE SERVICE ARTICLES - PROHIBITIONS
Observation: Pizza bags were improperly stored in the basement and were contaminated with dust and debris. These bags must be discarded.
Correction: Store single-service or single-use articles, clean equipment and utensils and laundered linens in areas that are not subject to contamination.
- FLOORS, WALLS AND CEILINGS - CLEANABILITY
Observation: The floor in the employee bathroom is raw wood planks and the walls are in poor condition.
Correction: Provide floors, walls, and ceilings that are designed, constructed, and installed so they are smooth and easily cleanable. Install 6 inch base cove in the bathroom.
- FLOOR CARPETING - RESTRICTIONS
Observation: Carpeting is installed under the mop sink.
Correction: Remove carpeting.
- INSECTS AND RODENTS - OUTER OPENINGS - SOLID, SELF-CLOSING AND TIGHT FITTING DOORS
Observation: The exterior door in the back room is not self-closing or tight-fitting.
Correction: Make the door self-closing and tight fitting to prevent the entrance of insects or rodents.
