Employee was observed using common towel on hands, as well as for wiping off food contact surface of preparation table in back area for preparation of pizza dough. A twist top bottle employee beverage was found stored next to the soft drink fountain. Pizza Marinara sauce, and ranch sauces were found stored in the Small noncommercial grade reach in food cooler at the end of the pizza oven at 48°F - 50°F. The manager stated that the marinara and ranch had been in the reach in cooler for an hour. The faucet on the 3 compartment sink did not have cold running water available, due to no handle for cold water was installed on the faucet. The faucet on the wall by the hand sink in the back pizza dough making area did not have a backflow prevent device installed on it such as an AVB (Atmospheric Vacuum Breaker). A bag of granulated sugar was found stored on the floor in the back area by the pizza dough preparation table. The small noncommercial grade reach in cooler did not have a thermometer installed in it. The table top mounted can opener blade was found soiled. The food utensils and pizza cutting equipment were found soiled in storage containers. The top of the pizza oven was found soiled. The filters and the vent hood above the pizza oven were found soiled. Food utensils were found with non-easily cleanable handles, the knife had electrical tape on the handle, and the putty spatula had a cracked and broken handle that was not smooth or easily cleanable.
