Violation descriptions and comments

Raw chicken was observed being stored on top of and next to ready to eat foods (raw carrots) on the bottom shelf of the reach-in refrigerator.
Uncovered employee beverage containers were present on the kitchen small table on the prep line. Two employee twist top soft drink bottles were found in the reach-in food cooler next to food items intended for service to customers.
Dead cock roaches were found in the rolling can rack, and food equipment shelf above the prep sink, in the dry storage cabinets inside of the front service line counter, and cock roaches were stuck in a trap in the front cabinets.

An uncovered employee beverage container was present on the small food preparation table by the fry line.
Cooked rice prepared at 9:00 AM on 8/27/13 was 80�°F at 2:00 PM after cooking for more than 2 hours. The rice was re-heated to 165�°F in the rice steamer.
At the time of the inspection, an old refrigerator was being taken out of the kitchen and was being replaced with two new refrigerators, and the following items were being held in the kitchen unrefrigerated: cooked rice was 78�°F, cooked chicken (chicken breast) was 86�°F, sweet and sour chicken was 67�°F, cooked beef was 102�°F, cooked chicken was 112�°F, four packages of egg rolls were 46�°F, shell eggs were 78�°F. Pooled eggs in the cooks' line cold top refrigerator were 54�°F.
Several live flies were observed in the facility and on cooked and fresh food items.

An uncovered employee beverage container was present on the main kitchen food preparation table, as food was being prepared.
Fried chicken was 71�°F, cooked broccoli was 68�°F, pooled eggs were 44�°F. The fried chicken and cooked broccoli were in uncovered metal and plastic containers and were not refrigerated. The pooled eggs were in the wok line cold top refrigerator.

Three employee beverage containers were present on the wok line food preparation table. An employee beverage in a twist off lid container was present on the top shelf of the single door stand up refrigerator in the kitchen. It was stored above food intended for customers.
Tongs were present in the front service line handsink, thus making it inaccessible for hand washing.
An unlabeled chemical spray bottle was present on the chemical shelf to the right of the ware-washing sink.

One severely-dented 6 lb 9 ounce can of bamboo shoots was stored with cans of wholesome food on the can rack.
Two open employee beverages in soda cans were present on the metal shelf directly above the kitchen cold top refrigerator and the kitchen food preparation table.
Raw chicken was 49�°F in the single door stand up refrigerator in the kitchen. Raw beef was 45.5�°F in the same cooler.
Two live cock roaches were observed in the kitchen, one was on the can rack and another was on the wall below the kitchen prep area handsink.
A bottle of rubbing alcohol was stored next to cans of food on the shelf above the small wok line food preparation table.

The operator did not have chlorine bleach or quaternary ammonia or iodine available for manual sanitization of food contact surfaces. Corrected on site, operator went to King Soopers and bought chlorine bleach.
Live cock roaches were present in several of the adhesive traps throughout the kitchen. Dead cock roaches were observed behind the stand up kitchen refrigerators, on the can rack, and on the utensil storage shelves above the prep sink and the three-compartment sink.

One live cock roach was present below the employee radio, one was on the floor below the kitchen prep table, and several dead cock roaches were present on the floor by the mopsink and on the floor below the south end of the wok line.
