Cooked chicken wings were 45-46�°F in a tightly covered plastic container in the kitchen main prep station reach-in refrigerator. Operator voluntarily condemned these chicken wings.
An unlabeled chemical spray bottle was present on the utility closet shelf. Corrected on site.

A package of cooked ham was stored on packages of raw ground beef in the walk in refrigerator. Corrected on site.
The consumer advisory on the menu did not identify the animal-derived foods by asterisking them to a footnote that states that these items may contain under cooked ingredients, and it did not state that regarding the safety of these items, written information is available upon request, and that consuming raw or undercooked meats, poultry, seafood, shellfish or eggs may increase your risk of foodborne illness, especially if you have certain medical conditions.
