Violation descriptions and comments

Raw beef was stored above raw pork in the walk-in refrigerator. Corrected on site.
The rice station reach-in refrigerator had worn plastic coating on the refrigerator shelves, and the exposed metal was rusted. Several metal pans stacked on the rice station prep table shelf had severely-dented corners, and the food contact surfaces were no longer smooth and easily cleanable. Taco salad shells were stored in unlined wicker baskets at the service line.
A couple of the shelves in the walk-in refrigerator had worn plastic coatings, and the exposed metal was rusted. Several sections of the three-compartment sink wall caulking were missing.
The ice machine ice deflector plate was soiled with a thin pink film. Numerous tongs hanging above the main food preparation table had food contact surfaces that were soiled with burned food residue.
The top of the water heater was soiled with debris. The door gasket on the small reach-in freezer by the office doorway was soiled with food debris. The pre-rinse sprayer nozzle was soiled with black grime.
The automatic ware washing machine dirty side drain board wall caulking was soiled with grime. Several washed food containers were stacked together while still wet, on the three-compartment sink drain board. Utensils were being washed in 102�°F water in the three-compartment sink.
Wiping cloths were stored in a wiping cloth bucket that had a concentration of less than 100 ppm quaternary ammonia at the front service line.
The floor drain below the automatic ware washing machine drained very slowly. The front service line handsink wall caulking did not completely seal the sink to the wall.
Several unsealed holes were present in the stainless steel wall surfaces of the mopsink area. The wall behind and below the automatic ware washing machine was soiled with greasy film.
The cooks line ventilation hood filter screens were installed with gaps larger than one-inch.
