The facility had bags of raw chicken in a metal pan being stored next to raw jalapenos on the bottom shelf of the walk in cooler. The inspector found 2 cups in the back dry storage area that were being stored in the cup holder, however the cups did not have lids on them or straws. The inspector also found a personal beverage cup being stored in the reach in cooler in the preparation area. The inspector found 6 containers of guacamole in reused sour cream containers that were at a range of 48°F - 50°F. The staff stated that the guacamole was made on 7/26/13, the guacamole was voluntarily discarded at the time of inspection. The inspector found a spray bottle that was not labeled, being stored with other spray bottles above the hand sink, and in the ware washing area. The wire shelves in the 6 door reach in food cooler in the cooking area had rust on the shelves. The table mounted can opener blade was soiled with food residue. The interior of the 6 door reach in food cooler had some food residue on the bottom of it. The inside of the 3 door reach in food cooler in the dry storage area had standing water in it. The whisk had dried refried beans inside of the wires/ next to the handle. The tops of the insides of the 3 stacked microwaves are needing to be cleaned due to buildup of food particles. There was food residue in the pans that were found to be stacked in the storage area next to the ware washing area. The inspector found dust on the racks in the walk in cooler. The large, and small cooking pots in the ware washing area, and the preparation area were found to have burned on food debris on them. The ceiling tiles, and the vent duct in the food preparation area need to be cleaned. The inspector found wet food pans being stacked together before completely drying. The facility was found using the sour cream containers and holding guacamole in them. The floor seal in the walk in cooler, and walk in freezer needs to be addresses as the floor is rusting. The light shields throughout the ware washing area, food preparation area, and cooking area need to be cleaned. The paint in the ceiling of the water heater room is peeling and chipping. The floor tiles are chipped on the cooking line by the flat top griddle. The ceiling above the hand sink in the cooking area has paint that is chipping and peeling. The light bulbs in the food waiting preparation area is needing to be replaced The back door in the dry storage area needs to have the seals replaced as light is able to be seen on the side and the bottom of the door frame.
